to the commencement of fieldwork. The Site-Specific HASP will be reviewed and signed by all field staff <br /> and contractors before beginning field operations at the site. <br /> Planning and Permitting <br /> Atlas will obtain a well permit from SJCEHD prior to initiating the field work. Additionally,for off-site wells, <br /> access agreements will be secured. Grouting activities will be performed under the direction and/or <br /> supervision of an appointed representative of the SJCEHD. <br /> Underground Utility Locating and Clearance <br /> In advance of field activities, Atlas will mark the locations of the existing wells in accordance with the <br /> Underground Service Alert (USA) guidelines, and notify USA of upcoming subsurface activities. Atlas will <br /> also contract a private utility locator to confirm the locations of underground utilities in the vicinity of <br /> each well. <br /> Proposed Well Destruction Activities <br /> The current groundwater monitoring well network consists of twelve on-site wells(MW-3, MW-4R,MW-7, <br /> MW-12R, MW-13, MW-14, MW-IS, MW-17, MW-19, MW-20, MW-21, and MW-22), and seven off-site <br /> wells (MW-5, MW-6, MW-9, MW-16, MW-23, MW-24, and MW-25). There are 3 soil vapor wells on-site <br /> (SVE-1 through SVE-3) and seven air sparge well (AS-1 through AS-7). Well locations are shown on Figure <br /> 2 and Figure 2A. <br /> Monitoring wells at the site are generally completed in two zones. Shallow wells(MW-4R, MW-14 through <br /> MW-17, MW-19,and MW-25) are screened between 25 and 40 feet bgs. The cross-screened well (MW-7) <br /> is screened between 26 and 56 feet bgs. Deep wells(MW-3, MW-5, MW-6, MW-12R, MW-13,and MW-20 <br /> through MW-24) are screened between 40 and 80 feet bgs. Well construction details are shown in Table <br /> 1. <br /> Atlas will contract with a C-57 drilling company to properly destroy the wells. Each of the wells will be <br /> cleared to five feet below ground surface utilizing air knife procedures. Each well will then be overdrilled <br /> to the depth of the original boring with an eight-inch minimum diameter hollow stem auger to ensure all <br /> well materials are removed. The boreholes will be grouted to the surface from the total depth up as the <br /> augers are removed from the subsurface using neat Portland I/II cement. <br /> Waste Management <br /> The excavated soil and overdrilled material will be generated and stored on-site in a soil bin. Once the <br /> soil is profiled, the bin will be transported by a licensed waste hauler and disposed of at an approved <br /> facility. <br /> Report Preparation <br /> Upon completion of the well destruction activities, a summary report will be prepared that will include a <br /> description of well destruction field activities associated with the site, including records of waste disposal. <br /> Well Completion/Destruction Reports will be submitted to the Department of Water Resources by the <br /> drilling company. <br /> SCHEDULE <br /> Atlas will notify the SJCEHD prior to commencement of field activities to arrange for inspections. <br />
