Maintain a copy on site. <br /> This is a Repeat(Class 2)Violation. <br /> 34 23 CCR 16 2631(a), (f)-a) 2031017 - DO inspections completed at least every 30 days and meet all requirements (TCR <br /> 12) <br /> OBSERVATION: Facility did not comply with one or more of the following DO inspection requirements: Conduct the DO <br /> visual inspection at least once every 30 days; Performed by an ICC certified DO; Inspect monthly alarm history report, <br /> check that alarms are documented and responded to appropriately, and attach a copy; Inspect for damage and for the <br /> presence of liquid/debris in spill containers; Inspect for damage and for the presence of liquid/debris in under dispenser <br /> containment (UDC) and ensure that the monitoring equipment is positioned correctly; Inspect for damage and for liquid or <br /> debris in containment sumps where an alarm occurred with no service visit; Check that all testing and maintenance has <br /> been completed and documented; The owner/operator signed and dated the Designated Underground Storage Tank <br /> Operator Visual Inspection Report within 72 hours of being provided a signed copy. <br /> -The "Name of UST Owner/Operator(print)" on page 2 of the DO inspection reports do not list a name. <br /> REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and shall check all required items on the report. <br /> CORRECTIVE ACTION: Ensure that facility is complying with all DO inspection requirements noted above. <br /> This is a Repeat(Class 2)Violation. <br /> 36 23 CCR 16 2631(b) and (c), and 2643(e) 2031018 - Initial and annual training provided to employees and at least one <br /> trained employee is present <br /> OBSERVATION: Owner/Operator failed to do one or more of the following: Provide initial training before the individual <br /> performs the duties of a facility employee; Train facility employees in the proper operation and maintenance of the <br /> underground storage tank system at least once every 12 months; Have at least one employee present during operating <br /> hours that has been trained in the proper operation and maintenance of the UST system by a designated operator(DO). <br /> -The most recent training date documented was on 1/28/25. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual <br /> performs the duties of a facility employees. The training shall include, but is not limited to: 1. Operation of the UST <br /> system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring <br /> equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in <br /> the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial <br /> training shall also include a site-specific practical demonstration that provides both sharing information and evaluating <br /> knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills.
