OBSERVATION:A release of 4,000 gallon spill occurred on 11/22/2024 and was not reported to the EHD and CalOES until 11-26-2024 four days later. REGULATION GUIDANCE:A release or threatened release of a hazardous material shall be verbally reported immediately to the EHD and CalOES. CORRECTIVE ACTION: Submit a written statement to EHD stating any future releases or threatened releases of a hazardous material will be reported immediately to the EHD and CaIOES./Submit employees training records that show annual/initial training and refresher courses. Training shall include familiarity with the facility's Emergency response plans and procedures in the event of a release or threatened release of a hazardous material as stated in the Health and Safety Code 25505(a)(4).
