For commodity fumigations,the term"Direct Supervision"is defined at minimum as a qualified applicator(holding a <br /> Department of Pesticide Regulation issued Qualified Applicator's Certificate(QAC)or Qualified Applicator's License(QAL) <br /> card, as the situation demands, in the subcategory"A")who is physically on-site during the application and initial aeration of <br /> the chamber. <br /> SITE CONDITIONS <br /> Appendix H Aluminum Phosphide and Magnes <br /> Aluminum Phosphide and Magnesium Phosphide for Burrowing Rodents <br /> See attached: Appendix H <br /> GWP-L1 No irrigation <br /> CONDITION -GWP-L1 -LEACHING GROUNDWATER PROTECTION AREAS. NO IRRIGATION. <br /> The permittee or his representatives shall not apply irrigation water for at least six(6)months after the application of any 3CCR <br /> section 6800(a)pesticide. <br /> GWP-L2 Bed or berm application <br /> CONDITION -GWP-L2-LEACHING GROUND WATER PROTECTION AREAS. BED OR BERM APPLICATION. <br /> If the permittee chooses to apply a pesticide listed on 3CCR section 6800(a), the resulting application to the planting bed or the <br /> berm must be above the level of irrigation water in the furrow or basin and the water level shall remain at or below that level for <br /> six(6)months following the application. <br /> GWP-L3 Managed irrigation <br /> CONDITION -GWP-L3-LEACHING GROUND WATER PROTECTION AREAS.MANAGED IRRIGATION. <br /> For six(6)months after the application of pesticides listed in 3CCR section 6800(a), the permittee shall manage irrigation so <br /> that the ratio of the amount of irrigation water applied divided by the net irrigation requirement is 1.33 or less. <br /> GWP-R3 Band treatment <br /> CONDITION -GWP-R3-RUNOFF GROUNDWATER PROTECTION AREAS. BAND TREATMENT. <br /> 3CCR section 6800(a)pesticides shall be applied as a band treatment immediately adjacent to the crop row so that not more <br /> than thirty-three(33)percent of the distance between rows is treated. <br /> P General Conditions <br /> CONDITION -P-General <br /> Applicators should consider viable alternative methods prior to any pesticide use. <br /> The following conditions apply to applications under this permit.Applicators must additionally follow all applicable laws, <br /> regulations, label restrictions and other more restrictive conditions of this permit. <br /> 1. Maximum wind velocities for applications of Restricted Materials unless otherwise covered by the label, law, regulation,other <br /> permit conditions or environmental conditions at the application site are: <br /> a. Spray- 10 m.p.h. <br /> b. Dust-8 m.p.h. <br /> c. Pellets,granular or baits- 16 m.p.h. <br /> 2. Unless specifically named as a dust on this permit, NO DUST FORMULATIONS are allowed. <br /> 3. Apply pesticides covered by SLN(s)and Section 18(s)in conformance with the SLN(s)and Section 18(s). The SLN and/or <br /> Section 18 must be present at the use site along with the registered label. <br /> 4.A flagger is required at the application site for all AIR applications of Zinc Phosphide bait. <br /> 5.The applicator shall submit Notice of Intents(NOls)for any California Restricted Material to be used. NOls will only be <br />
