Installation of the traffic-rated 1,200-gallon P&L Concrete, Inc., septic tank installed adjacent to <br /> the handicap parking stall as illustrated in the proposed site map. Installation of the three (3) hole <br /> d-box with minimum 5 feet tight line coming from the septic tank. <br /> 5. Leach line installation measuring 100 feet long following EHD OWTS Standards coming from the <br /> three-hole d-box for the primary disposal field installed in the 10-foot-wide landscape strip running <br /> parallel to Grove Tree Lane to the north. As stated in the report, the leach line area shall be <br /> barricaded by a curb on the east side and a chain-linked fence on the west side of the leach line <br /> area. <br /> 6. As mentioned in the report, the site should consider installing the disconnected tight line south of <br /> the d-box running under the proposed driveway in the initial septic installation permit to prevent <br /> disturbing the driveway for when the 100% replacement area is needed. The 100% replacement <br /> area will be identical to the primary disposal field area and following EHD OWTS Standards. <br /> 7. Obtain an installation permit for the onsite wastewater treatment system (OWTS) from EHD. <br /> Inspection is required before backfilling of OWTS excavations. Request inspection 24 hours in <br /> advance by calling the inspection line (209) 953-7697. (SJC OWTS Standard Section 2) <br /> 8. It is the owner's responsibility to follow operation and maintenance plan to ensure the system <br /> meets all requirements set forth in the design in conformity with San Joaquin County (OWTS) <br /> Standard. Below are the main points listed in the Operations and Maintenance Plan described in <br /> the report: <br /> a. Fixtures should be checked in each restroom to ensure there is no leakage on a weekly <br /> basis. <br /> b. Two years after operation, the septic tank should be inspected by a qualified professional <br /> to determine if the tank should be pumped. <br /> c.
