Food Program Service Request Inspection Report <br />Facility Name and Address: SAKANA JAPANESE/CHINESE BISTRO, 1460 W YOSEMITE AVE , MANTECA 95337 <br />Environmental Health Department <br />Floor, base, walls and ceiling at the front and back of the house are complying with codes. <br />A designated area for janitorial equipment and mop sink , by walk in cooler, is provided. <br />Metal splash guard provided at the back hand sink. <br />Ice maker, walk in cooler, prep sink, 3 comp sink and all other sinks are draining with indirect connection and air gap. <br />Gas water heater of 75,100 BTU is installed and more than the minimum requirement. <br />****Provide valid food manager certificate within 60 days of operation and rest of employees food handler cards within 30 days <br />from hiring dates. Keep copies on site at all times. <br />Okay to operate. <br />Obtain permit prior operating your business. <br />Seats 36 <br />PE1624 $373 permit fee. <br />Available credit balance of $179 paid in excess in August 14th 2025 with confirmation# 207079313. <br />Remaining balance to be paid is $194 + $15 technology fee to be paid for the new permit. <br />5021 form to be updated. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
