HSC 117975(a)(3) (a) A large quantity generator required to register pursuant to this chapter shall maintain for a <br />minimum of two years individual treatment records and the tracking document for all untreated medical waste shipped <br />offsite for treatment. The generator shall report or submit to the enforcement agency, upon request, all of the following: <br />(3) Tracking documents or electronically archived tracking documents maintained by the facility or medical waste <br />hauler of all untreated medical wastes shipped offsite for treatment. <br />OBSERVATIONS: <br />1. Tracking documents for the following dates did not have the generator information completed: 12/31/24 and 6/13/25. <br />2. Driver did not contact a generator representative to sign off on the medical waste tracking document dated 6/13/25. This <br />tracking document also included a date discrepancy with the date next to generator and hauler information being <br />6/17/25. <br />CORRECTIVE ACTIONS: <br />1. Implement facility procedures to ensure that all staff are aware of the proper steps needed to maintain tracking <br />documents of medical waste shipped off site. In situations where the medical waste hauler does not contact anyone <br />within the facility to sign off on medical waste tracking document, contact the medical waste hauler to discuss and <br />implement procedures to ensure that generator staff are able to sign medical waste tracking documents. <br />2. Generator is to train all staff personnel to sign off on all medical waste tracking documents and to verify that tracking <br />documents are being completed in full and correctly by both the generator and the driver to prevent any future <br />discrepancies. <br />25. HSC 118280(e)(1)(A) To containerize biohazard bags, a person shall do any of the following: (e)(1) Except as <br />provided in paragraph (5), a person generating biohazardous waste shall comply with the following requirements: (A) if <br />the person generates 20 or more pounds of biohazardous waste per month, the person shall not contain or store that <br />waste above 0 degrees Centigrade (32 degrees Fahrenheit) at an onsite location for more than seven days without <br />obtaining prior written approval of the enforcement agency. <br />OBSERVATIONS: <br />1. Previous pick-up on 12/20/25. Regularly scheduled pick-up for 12/27/24 was delayed due to the holidays. This was <br />noted in the manifest binder. Pick up was scheduled with the medical waste hauler for 12/31/24. This is an 11 day <br />stretch where medical waste was stored onsite at or above 0 degrees centigrade. <br />2. Previous pick-up on 6/27/25. Regularly scheduled pick-up for 7/4/25 was delayed due to the holiday. This was noted in <br />the manifest binder. Pick up was scheduled with the medical waste hauler for 7/7/25. This is a 10 day stretch where <br />medical waste was stored onsite at or above 0 degrees centigrade. <br />3. Previous pick-up on 7/25/25. Regularly scheduled pick-up for 8/1/25 was delayed due to staffing issue. Within manifest <br />binder there was a sheet with notes from Danette Belletto noting that medical waste hauler was called and they were <br />notified due to staffing issues that driver will be available for pick up. Pick up was scheduled for 8/2/25. <br />4. Date discrepancy found on tracking document dated 6/13/25. Generator and hauler date showed 6/17/25 <br />CORRECTIVE ACTIONS: <br />1. Ensure that medical waste is not being stored on site for more that 7 days. <br />Corrections due to EHD by December 15, 2025 <br />
