Payment receipt was shown <br />during inspection. <br />5. Medical waste hauler for facility is MediWaste. <br />6. Training records on the completion of medical waste tracking documents were obtained for 11/4/24 and 8/18/25. <br />7. Biohazard bags on-site were reviewed and are ASTM D1709 and ASTM D1922 <br />
