Remedial Investigation QAPP <br /> Stockton Army Aviation Support Facility,California <br /> The Field Team Lead will have completed the 8-hour OSHA Supervisor training,as specified in EM-385.The Field <br /> Team Lead will lead field operations, coordinate field activities, and act as the liaison between Facility and <br /> laboratory personnel,among other responsibilities. <br /> In general, field personnel will wear PFAS-free Level D personal protective equipment (PPE). Additional <br /> information regarding appropriate PPE and personal products (e.g., bug spray) are provided in Parsons SOP <br /> PFAS-01: PFAS Sampling Guidance (Appendix D). If elevated levels of PPE are warranted based on Facility <br /> conditions,it will be established in the site-specific SSHP.Detailed activity hazard analysis(AHA)forms identifying <br /> the physical, chemical, and biological hazards that may be encountered at the Facility and the associated <br /> mitigation methods are presented in the APP/SSHP. <br /> Personnel and visitors who enter the Facility will be required to review the APP/SSHP and sign the <br /> acknowledgement form. Site workers will be required to sign the daily tailgate safety meeting form and fill out <br /> daily AHA forms.Safety issues that arise during implementation of field activities will be addressed duringtailgate <br /> safety meetings held daily before the workday and will be documented in the daily tailgate safety meeting form. <br /> 14.1.3 Additional Personnel Qualifications <br /> In addition to the health and safety requirements specified above, personnel mobilized to the Facility will <br /> complete the DoD's Operations Security Awareness for Military Members, DoD Employees, and Contractors and <br /> Level 1 Antiterrorism Awareness Training. Drilling and land surveying subcontractors will be licensed in the state <br /> of California. In compliance with San Joaquin County Well Ordinance(San Joaquin County, 2005),all monitoring <br /> wells and borings with a total depth greater than 10 ft will be performed under the direct supervision of a <br /> California Registered Professional Engineer, California Registered Geologist, or California Certified Engineering <br /> Geologist. <br /> 14.1.4 Utility Clearance, Permits, and Notifications <br /> Utility clearance is required for intrusive work, regardless of planned intrusive depth. Prior to intrusive activities, <br /> a site walk will be scheduled with the appropriate ARNG personnel and personnel from other entities identified <br /> by ARNG, as applicable, to mark out locations of the subsurface utilities. The location of utilities will be noted <br /> and recorded during the site visits and referenced when selecting investigation locations. Parsons or its drilling <br /> subcontractor will contact California One Call (811),the local one-call utility location system at least 48 hours in <br /> advance of the field activities. Precautionary measures (e.g., geophysical survey, air knifing methods, hand- <br /> digging to 5 feet, etc.) are required for all boring/well locations prior to intrusive activities. Utility clearance will <br /> be conducted in accordance with Parsons SOP PFAS ENV-08:Utility Clearance(Appendix D). <br /> San Joaquin County requires that all monitoring wells and geophysical test holes comply with San Joaquin County <br /> Ordinance Code Section 9-1115.6, Well Standards (San Joaquin County, 2005). A permit application(s) will be <br /> completed by a licensed well contractor with the San Joaquin County Environmental Health Department for all <br /> monitoring well installations,soil borings, and monitoring well closures. <br /> Due to the proximity of the Stockton Municipal Airport,a Federal Aviation Administration(FAA)Notice of Proposed <br /> Construction or Alternation (Form 7460-1) may be required 45-days in advance of drilling activities. However, in <br /> order to allow ample time for the FAA-approval process, this will be submitted as soon as the sampling plan is <br /> finalized, well in advance of the 45-day FAA requirement. Any required coordination with FAA will be conducted <br /> by ARNG (with support from the Parsons Team, as needed), with consideration given to coordinating fieldwork <br /> with any scheduled runway shutdowns that may be planned.As required in Form 7460-1,the relationship of the <br /> planned fieldwork activities will be marked with a precise indication of the locations on a 7.5-minute US <br /> PARSONS Worksheet#14&16: Project Tasks and Schedule <br /> Page 62 <br />
