SA N J O A Q U I N Environmental Health Department <br /> COUNTY <br /> ADDITIONAL HAZARDOUS SUBSTANCE REQUIREMENTS: <br /> 9. If the removed backfill material and soil is found to be contaminated it must be disposed of at <br /> an approved disposal location. A complete hazardous waste determination must be made in <br /> accordance with CCR Title 22 regulations prior to disposal. Treatment of soil on site can only <br /> be performed under a hazardous waste treatment permit. <br /> 10. Any compactable material imported onto the site for the purpose of backfilling must be clean <br /> compactable backfill. <br /> 11. The discovery of any released hazardous substance at the site of the UST(s) or in the <br /> surrounding area, including the presence of free product or vapors, must be recorded in on site <br /> records and reported to the EHD in accordance with CCR Title 23 Article 7 release reporting <br /> requirements within 24 hours after an unauthorized release or condition has been detected, or <br /> should have been detected. <br /> SUBMIT WITHIN 30 DAYS OF THE REMOVAL: <br /> 12. The following information must be submitted within 30 days of the tank or piping removal date: <br /> a. Analytical results: Laboratory reports containing the sample analytical results, chain- <br /> of-custody, quality assurance/quality control data, and any commentary or notes from <br /> the laboratory. Only sample analytical results that are sent to EHD directly from a third- <br /> party California accredited environmental laboratory with affixed chain of custody will be <br /> accepted. <br /> b. Sampling information: Site maps or drawings identifying the location at the facility, <br /> depth below grade and date of collection for each sample taken. <br /> c. Boring logs, if applicable. <br /> d. Disposal records: Documentation demonstrating proper disposal of the tank(s), piping, <br /> and any hazardous wastes generated as a result of closure activities, including: <br /> • Hazardous Waste Tank Closure Certification form (DTSC Form 1249) <br /> • Hazardous waste manifest(s)for all hazardous wastes generated, including <br /> tank(s), piping, rinsate, and residual product <br /> • UST Disposition Tracking Record for each tank not hauled using a hazardous <br /> waste manifest <br /> 13. The owner or operator shall log into the California Environmental Reporting System (CERS) at <br /> http://cers.calepa.ca.gov/ and update all applicable data elements within 30 days of removal of <br /> the UST from the excavation. <br /> RECORDS MAINTENANCE: <br /> 14. The owner or operator must maintain the analytical results of all soil and groundwater samples <br /> for at least 36 months after the UST system is properly closed. <br /> ADDITIONAL BILLING: <br /> 15. The permit application included payment for 3 hours of time. Additional time may be required for <br /> inspections and review to verify that this UST work complies with all applicable laws and <br /> regulations. All EHD staff time spent beyond the prepaid amount will be billed to the applicant at <br /> the current EHD hourly rate ($179). <br /> 3 of 4 <br />
