EHD was not <br /> notified of this work, a "Failure to obtain a Permit" penalty applies. <br /> REGULATION GUIDANCE: Failure to obtain or retain a valid permit to operate from the UPA and comply with the <br /> applicable requirements of the permit, including but not limited to paying all applicable local government fees and state <br /> surcharge fees. <br /> CORRECTIVE ACTION: If Owner/Operator has not obtained a valid permit to operate, or if the permit to operate has <br /> expired, a complete application to obtain a current permit to operate must be submitted to the UPA for approval. If facility <br /> is not in compliance with one or more requirements of the permit to operate, owner/operator must submit documentation <br /> of compliance. All unpaid permit fees must be paid.
