EHD was not <br /> notified of this work, a "Failure to obtain a Permit" penalty applies. <br /> REGULATION GUIDANCE: Failure to obtain or retain a valid permit to operate from the UPA and comply with the <br /> applicable requirements of the permit, including but not limited to paying all applicable local government fees and state <br /> surcharge fees. <br /> CORRECTIVE ACTION: If Owner/Operator has not obtained a valid permit to operate, or if the permit to operate has <br /> expired, a complete application to obtain a current permit to operate must be submitted to the UPA for approval. If facility <br /> is not in compliance with one or more requirements of the permit to operate, owner/operator must submit documentation <br /> of compliance. All unpaid permit fees must be paid. All applicable violations must be corrected within 30 days. <br /> This is a Repeat(Class 2)Violation. <br /> 4 23 CCR 16 2613(a)(4) and (b), 2650(a), (c), and (e) 2011012 - Facility has submitted and maintains a monitoring site <br /> plan <br /> OBSERVATION: Owner/Operator failed to submit and maintain a monitoring site plan for approval by the UPA. <br /> -The Monitoring Site Plan is not drawn to scale and does not show the location of the piping. <br /> REGULATION GUIDANCE: Failure to submit and maintain a monitoring site plan for approval by the UPA. <br /> CORRECTIVE ACTION: Submit and maintain a monitoring site plan. <br /> This is a Repeat(Class 2)Violation. <br /> 19 23 CCR 16 2651(c)(1) 2031006 - Leak detection equipment properly installed, calibrated, operated, and maintained <br /> (TCR 9d) <br /> OBSERVATION: Owner/Operator failed to properly install, calibrate, operate and/or maintain leak detection equipment in <br /> accordance with manufacturer's specifications. <br /> -The Diesel LLD was unable to be tested due to electrical issues according to service technician Zane Nimmo. <br /> REGULATION GUIDANCE: "Failure of the release detection equipment to be installed, calibrated, operated, or <br /> maintained in accordance with manufacturer's specifications. <br /> CORRECTIVE ACTION: Properly install, calibrate, operate and/or maintain leak detection equipment in accordance with <br /> manufacturer's specifications.
