Job Steps Risks/Hazards Quality/Safe Work Practices <br /> Perform wrong test/job function Review work/sampling plan,JLA, laboratory instructions, project task <br /> Perform task according to approved assignment,and other directions before performing task <br /> plan/procedure <br /> Miss a step or make a mistake while performing activity Have copy of procedure/standard available while performing task <br /> Pack bottles in cooler in adherence with laboratory instructions to avoid <br /> Sample bottle breakage breakage or compromising cooler integrity(i.e.,use of plastic bags/baggies, <br /> cardboard or fabric separators,bubble wrap,packing tape,etc.). <br /> Depending on how the samples are being delivered to the laboratory(personal <br /> drop off,on-site/office laboratory courier,delivery service)different <br /> Samples do not arrive at laboratory as intended arrangements may need to be considered. Make sure to sign off samples to <br /> Sample packaging and shipment receiver on chain of custody. Consider estimated delivery time frames and third <br /> party delivery service shipping requirements that need to be followed(to avoid <br /> leaking coolers) <br /> Take the time to note the log-in requirements for laboratory sample receiving <br /> Failure to meet sample log-in requirements established by laboratory or and for each analytical method and assure that each requirement can be met; <br /> analytical method including, holding/handling times,cooler temperature,chain of custody <br /> documentation completion,etc. <br /> Page 4 of 4 <br />
