Stop - When an employee perceives conditions or behaviors as defined <br /> above (reference KQP-3.4, Section 6.1), he or she must immediately <br /> initiate a stop work intervention. <br /> 2. Report— Kleinfelder employees and subcontracted personnel are <br /> empowered and required to report a stop work condition to their immediate <br /> supervisor, as well as to any affected personnel. (Reference KQP-3.4, <br /> Section 6.2) <br /> 3. Investigate —The immediate supervisor shall immediately assess the stop <br /> work condition, issue verbal stop work orders, and engage a management <br /> official to determine if an official SWO will be issued. If the management <br /> official determines that an SWO is not required, they will document their <br /> decision and rescind the verbal SWO. If the management official <br /> determines that an SWO is required, they will communicate the SWO to the <br /> stakeholders (listed in Section 6.5 of KQP-3.4) and document the SWO. <br /> Resolution of the SWO requires investigation and determination of the root <br /> causes and corrective actions. (Reference KQP-3.4, Sections 6.2-6.5) <br /> 4. Document—The management official will document the SWO and <br /> distribute it in accordance with KQP-3.4, Sections 6.6 and 6.7. <br /> 5. Correct - Corrective actions will be made according to the corrections <br /> agreed upon in the investigation. Work shall not resume until the SWO has <br /> been rescinded. (Reference KQP-3.4, Sections 6.8-6.9) <br /> 6. Resume -All affected employees will be notified of the corrective actions <br /> implemented. Work will not resume until all stop work concerns have been <br /> addressed, and the management official has determined that the imminent <br /> risk has been eliminated or adequately mitigated, and the SWO has been <br /> rescinded. (Reference KQP-3.4, Section 6.10) <br /> 7.
