A <br />hazardous waste generator shall not treat, store, dispose of, transport or offer for transportation, hazardous waste without <br />obtaining and maintaining an EPA ID number. To maintain an active Identification Number you must annually complete <br />the Electronic Verification Questionnaire (eVQ). <br />- The EPA ID number CAL000360592 expired 6/30/2017 and has not been reactivated. <br />25205.16 (b)Any generator, transporter, and facility operator who fails to comply with <br />this section, or who fails to provide information required by the department to verify the accuracy of hazardous waste <br />activity data, shall be subject to suspension of any and all identification numbers assigned to the generator, transporter, <br />or facility operator and to any other authorized enforcement action. <br />CORRECTIVE ACTION: Submit documentation to the CUPA demonstrating that you have obtained an Identification <br />Number, or taken necessary steps to reactivate an inactive number. Ensure that you complete the annual Electronic <br />Verification Questionaire to maintain an active Identification Number. <br />This is a (Class 2) Violation. <br />1868 E.
