IN <br />7 <br />RECEIVED <br />AUG 10 2a <br />SAN J(JAUUIN COUNTY <br />Provide the last drill that your emergency responders conducted. OFFICE OF EMERGENCY SERVICES <br />The field exercise conducted during the time of the training is listed on the sign <br />off sheet. It included failure of a CL2 flex line, failure from the bi-gas tank, hose <br />leak from hydrochloric railcar, fuse plug failure on chlorine ton container. <br />Provide the most current fit test for responders. <br />I am providing the sign off sheets for the respiratory protection training conducted <br />during the Hazwoper refresher training. It has the fit test report. <br />Provide written respiratory program. <br />A copy is attached. <br />Managers /supervisors should be trained on all safety equipment (chlorine <br />kits). <br />Supervisors, Juan Huizar, Juan Larios, Paul Flores have been trained on the <br />installation of chlorine kits. Facility Manager Pat Cardiasmenos has not been <br />trained at this date. <br />Facility Map Complete and Accurate. <br />Additional chemicals were identified during the inspection. An updated <br />facility site map is required. <br />The map has been updated by adding the additional chemicals to the inventory <br />along with their location. Copies are enclosed. <br />Chemical Inventory Complete and Accurate. <br />Chemicals that were missing from the inventory have been added. Chemicals that <br />were packaged in sizes, or in containers that were not consistent with the <br />inventory were revised. Copies are attached. <br />10. Plant Operations Appear Safe. <br />Eye wash station by truck fill area. Cover cap gone, leaves in bowl, part of <br />the bowl is broken off. <br />The cover caps have been replaced, bowl has been cleaned and a new bowl has <br />been ordered. <br />Tank Farm area; numerous continues leaks of caustic are going into the <br />secondary containment. <br />
