<br />The Business Owner/Operator Identification form was not completed with all the accurate information.
<br />Business answered "NO" in CERS for storing on site reportable quantities of hazardous materials. Also
<br />the the facility answered "NO" for having CalARP regulated substances on site. Rhonda Rallios is no
<br />longer working for this business, and she is listed as the environmental and secondary emergency
<br />contact in CERS. These information are inaccurate. This information must be current and submitted as
<br />part of the business plan. Immediately log into the California Environmental Reporting System (CERS) at
<br />http://cers.calepa.ra.gov/, enter the correct or updated information, and submit to the EHD for approval.

<br />The Hazardous Materials Business Plan has been updated and submitted on 2/13/2018. The facility has

<br />Any material that meets or exceeds the reportable quantity shall be reported in the facility's business
<br />plan Immediately log into the California Environmental Reporting System (CERS) at
<br />http://cers.mlepa.ca.gov/, add the materials listed below to the hazardous material inventory, and submit
<br />to the EHD for approval. 957 gallons of Quorum Yellow P, 235 gallons Lift III, 1,356 gallons of Liquid 90 ,
<br />500 gallons of Quorum Clear V, 159 gallons of Oxonia (Perasan A), 76 gallons of Questar LAC, 1,465
<br />pounds of Sterilex Ultra Step, 1,640 pounds of Sani Step, 55 gallons of Boost 3200, All the following
<br />chemicals were found on site and have not been reported. 75 gallons of Defoamer FG, 1000 gallons of
<br />APC Aqua Chlor, 55 gallons of Alpet D2, 75 gallons of 10023, 65 gallons of 7009, 55 gallons of General
<br />Purpose Degreaser, 2, 700 gallons of Smart Wash Original, 55 gallons of Smart Wash Organic, 1,150
<br />pounds of Anhydrous Ammonia, 55 gallons of Used Oil, 55 gallons of Used Glycol, 55 gallons of Used
<br />Oil contaminated with Ammonia, 165 gallons of Hydrochloric Acid, 110 gallons of Refrigeration Oil, 3000
<br />gallons of of TEK-PG Glycol, 770 gallons of Cryogenic Nitrogen, 220 gallons of Cryogenic Oxygen, 200
<br />pounds of Nitrous Oxide, 1,245 cubic feet of compressed Oxygen gas, 354 cubic feet of Acetylene
<br />compress gas, 55 gallons of Organic Peroxide and 500 gallons of Propane.

<br />The Hazardous Materials Business Plan has been updated and submitted on 2/13/2018 to reflect the
<br />chemicals observed during the 1/24/2018 CUPA inspection. The facility has answered \"YES\" to having
<br />a CalARP regulated substance and for storing on site reportable quantities of hazardous materials in
<br />CERS.

<br />CUPA Inspection, Initial Site Visit, and Incident Investigation
<br />A site map was not completed and submitted as part of the business plan. A site map shall contain a
<br />north orientation, loading areas, internal roads, adjacent streets, storm and sewer drains, access and
<br />exit points, emergency shutoffs, evacuation staging areas, hazardous material handling and storage
<br />areas, and emergency response equipment. If a site map element is not applicable for your facility then
<br />list it on the map and label as "NA". Immediately log into the California Environmental Reporting System
<br />(CERS) at http://cers.ralepa.ca.gov/, upload the correct or updated information, and submit to the EHD
<br />for approval.
<br />Resolution
<br />The Hazardous Materials Business Plan has been updated and submitted on 2/13/2018 including an
<br />attached site map with all the necessary elements.

<br />The emergency response procedures were not submitted to CERS as part of the business plan . A
<br />business plan shall include the following emergency response procedures for a release or threatened
<br />release of hazardous materials, including, but not limited to, the following: - immediate notification of
<br />local emergency personnel and the Environmental Health Department (EHD) - procedures for the
<br />mitigation of a release or threatened release to minimize any potential harm or damage to persons,
<br />property, or the environment - evacuation plans and procedures, including immediate notice, for the site
<br />Immediately log into the California Environmental Reporting System (CERS) at
<br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for
<br />approval.
<br />Resolution
<br />The facility has developed an Emergency Action Plan and coordinated the response efforts with the local
<br />fire department via written letter to the fire department which included a copy of the Emergency Action
<br />Plan and the Facility Evacuation Map. Included in the 2/13/2018 HMBP submission was an Emergency
<br />Action Plan and Site Map.
