Item 603(604)—No facility diagram or didn't show location and contents of containers, transfer <br /> stations, and pipes. <br /> + Inclusion of a facility diagram in the SPCC Plan is not a requirement for a Tier I Qualified Facility <br /> when using the Tier 1 template. <br /> Item 605— Plan failed to address facility layout, operations discharge prevention methods and <br /> containers. <br /> • Please reference enclosed SPCC Tier 1 plan, page 4, Table G-4 for current inventory of containers <br /> with potential for an oil discharge, type of failure, potential discharge volume, flow direction, <br /> secondary containment method, and secondary containment capacity. <br /> Specific issues called out in inspection: <br /> o Site refrigeration oil drum storage at the lube shop has been removed. <br /> 0 500 gallon used oil tank removed and replace with 360 gallon used oil tank is listed in the <br /> plan. <br /> Item 612 (609)— Failed to provide secondary containment, diversionary structures, or equipment to <br /> prevent discharge. <br /> • The 250 gallon diesel emergency fire pump has secondary containment within the fire pump house. <br /> Item 618 (613)— Failed to conduct inspection or maintain records for three years. <br /> • Inspections will be conducted per the SPCC Plan and records maintained for three years. <br /> Item 620 (615) - Failed to designate a person accountable for discharge prevention <br /> • The Maintenance Manager is designated and accountable for discharge prevention. <br /> Item 621 (616) - Failed to schedule and conduct discharge prevention briefings at least annually <br /> • Discharge prevention briefings are conducted for oil handling personnel annually to assure adequate <br /> understanding of the SPCC Plan. <br /> Item 706- Failed to provide and maintain adequate secondary containment <br /> ■ The 250 gallon diesel emergency fire pump has secondary containment within the fire pump house. <br /> Item 711 (713)- Failed to perform scheduled tank tests and inspections by appropriately qualified <br /> personnel <br /> • SPCC Tank Inspections conducted will be completed by August 31, 2018. <br /> Item 714(716)- Failed to provide each container with a high level monitoring device <br /> • Only manual container filling operations are conducted. Filling operations for the used oil drums are <br /> monitored by the operator. <br /> Item 715(717)- Failed to regularly test liquid level sensing devices to ensure proper operation <br /> • The liquid level sensing devices are inspected on a monthly basis. <br /> 2 <br />
