Manifests or bills of lading for spent lead acid batteries for the last three years were not found on <br /> site. A copy of each bill of lading must be kept on site for at least three years. Immediately locate a <br /> copy of all bills of lading for used oil filters for the last three years, maintain them on site, and submit <br /> copies to the EHD by 07/13/2014. <br /> 128. Bills of lading for the recycling of used oil filters for the last three years were not found on site. A <br /> copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br /> bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the <br /> EHD by 07/13/2014. <br /> 605 & 701. Missing, unreadable, or incomplete hazardous waste information on one 55-gallon drum of <br /> floor sweep, one 55-gallon drum of solid waste, and two 55-gallon drums of used oil and waste <br /> antifreeze, respectively. All hazardous waste containers shall be marked with the following information: <br /> 1. the words "Hazardous Waste"; 2. name and address of generator; 3. hazardous properties <br /> 4. physical state; 5. composition (contents); 6. accumulation start date <br /> Immediately label these containers and ensure that all containers are marked with all the required <br /> information. Since these containers have been on site longer than 180 days, immediately contact a <br /> licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the <br /> manifest to the EHD by 07/13/2014. <br /> 804,808,809. Unable to determine the length of storage of four Universal Waste fluorescent lamps not <br /> stored in a container on the shop floor and one on the second floorr. Universal Wastes, including <br /> aerosol cans, lamps, alkaline batteries, mercury containing switches, CRT, electronic devices, etc. shall <br /> be stored in a closed container that prevents breakage and release to the environment, and the waste <br /> containers shall be marked to identify the type of Universal Waste and demonstrate the initial date of <br /> accumulation. Universal waste shall be managed within one year of storage, and a copy of the disposal <br /> receipt for the waste be retained on site for at least 3 years. Immediately dispose of the above <br /> Universal Waste - lamps and submit a copy of the disposal receipt to EHD by 07/13/2014. A pamphlet <br /> for the Small Hazardous Waste Disposal Facility has been supplied to the generator during this <br /> inspection <br /> 812. Copies of Universal Waste disposal records were not found on site.
