Page 3-5 of section 3.1.2 Inspection and <br /> Maintenance Checklists states "At the end of each month, a copy of the Stage II Vapor Recovery Inspection Log will <br /> be faxed to the Environmental Compliance Specialist at (865) 297-1283 or e-mailed to envsery@pilottravelcenters.com <br /> and the original will be retained in Appendix B for compliance inspections."The originals were not in Appendix B. <br /> Inspections and tests must be conducted in accordance with the written procedures developed in the Spill Prevention, <br /> Control, and Countermeasure (SPCC) Plan. Records of these inspections and tests must be signed by the <br /> appropriate supervisor or inspector and kept on site with the SPCC Plan for a period of three years. Immediately <br /> locate a copy of all inspection and testing records for the last three years, maintain them on site, and submit copies <br /> to the EHD. <br /> This is a Class II violation. <br /> 614 CFR 112.7(0(1) Failed to train personnel on discharge prevention. <br /> Oil handling personnel were not trained. Page 3-7 of section 3.2.1 Employee Training states "Discharge prevention <br /> briefings will be scheduled at least once a year. Individual responsibility is stressed, with particular emphasis on the <br /> immediate notification of the appropriate personnel. All personnel are required to acknowledge understanding of the <br /> minimum requirements of the SPCC rule by signing the Personnel Training Sign-off Sheet included in Appendix A." <br /> This Personnel Training Sign-off Sheet in Appendix A was not signed. At a minimum, oil handling personnel shall be <br /> trained in the operation and maintenance of equipment to prevent discharges; discharge procedure protocols; <br /> applicable pollution control laws, rules, and regulations, general facility operations, and the contents of the Spill <br /> Prevention, Control, and Countermeasure Plan.
