Failed to comply with new HW tanks stem assessment requirements. <br /> N A tank assessment was not available on site for the used oil tanks. <br /> Retained the services of Condor Earth Technologies, to review and update <br /> our SPCC Plan. A draft SPCC Plan was provided to MUD by Condor on <br /> R July 11, 2011 and is under review. Condor is in the process providing <br /> estimates for performing Hazardous Waste Tank assessments. <br /> 45 RV 45. Contingency Plan incomplete. <br /> The Contingency Plan for this facility did not include descriptions of <br /> N emergency response to fire or explosion, a list of emergency equipment at <br /> the facility, or the name, address, and phone number of the facility <br /> Emergency Coordinator. <br /> R Retained the services of Condor Earth Technologies, who are in the process <br /> of drafting the update of Hazardous Waste Contingency Plan. <br /> 47 RV 47. No emergency coordinator. <br /> N No Emergency Coordinator has been identified for this facility. <br /> Emergency coordinator is Chief Plant Operator, Fermin Garcia; contact <br /> R information is listed on (draft) 1st page of Contingency Plan and all HMMP <br /> documents for the Main and Tertiary Plants. The Final Contingency Plan <br /> will be forwarded when complete. <br /> 23 23. Failed to (1) Drain of free-flowing oil, (2) Reclaim metal, (3) Close <br /> V container w/initial date of accumulation, Label "drained used oil filters", (4) <br /> Store <1 ton/< 1 ear, 5 Retain bill of lading for filters for 3 years. <br /> N Bills of lading for the recycling of used oil filters for the last three years were <br /> not found on site. <br /> R Bills of Lading for last 3 years are being researched and located. Will <br /> continue to provide follow-up until complete. <br /> 35 V 35. Failed to conduct daily inspections of HW tanks. Addressed and <br /> under SJEHD Review. <br /> 49 V 49. Personnel Training Records incomplete. <br /> N A written training program and training records were not available on site. <br /> Attached are available HazMat/HazWaste/EmergencyResponse training <br /> records. The necessary updates and improvements to the current <br /> HazMat/HazWaste/EmergencyResponse Training Plan are underway. The <br /> R Stockton Regional Wastewater System Emergency Response Plan is under <br /> revision, A June 30, 2011 draft is being reviewed by MUD senior <br /> management. The written HazMat/HazWaste/Emergency Response training <br /> program will be updated and keyed to this Emergency Plan. Will continue to <br /> provide follow-up until complete. <br /> 59 V 59. Failed to send generator manifests copies to the Department within 30 <br /> days. Addressed and under SJEHD Review. <br />
