Failed to keep signed copy for 3 years. 61. Failed to determine status <br /> 61 V of hazardous waste when manifest copy not received. 62. Failed to submit <br /> & an exce tion report. <br /> 62 Manifest numbers 004907611JJK (3/26/09), 004907722JJK (7/21/09), <br /> N 004907827JJK (10/14/09), 004907917JJK (1/14/10), 006396647JJK <br /> (6/17/10), 006396849JJK (12/30/10) and 006661230JJK (3/24/11) were <br /> found without a signed off copy from the destination facility. <br /> Manifest numbers 004907611JJK (3/26/09), 004907722JJK (7/21/09), <br /> 004907827JJK (10/14/09), 004907917JJK (1/14/10), 006396647JJK <br /> (6/17/10), 006396849JJK (12/30/10) and 006661230JJK (3/24/11) are all <br /> Manifests for disposal of Non-RCRA Solids, oily waste that were transported <br /> R by Evergreen Environmental. Inquiries have been made Evergreen <br /> Environmental to locate the designated facility signed copy of the manifests. <br /> We have requested an update from Evergreen on their progress on locating <br /> these manifests. If they are unable to locate the Manifests by August 29tH <br /> Exception reports will be filed with DTSC. <br /> 60 V 60. Failed to keep signed copy for 3 years. 63. Failed to keep a copy of <br /> & the consolidated manifest for 3 years. <br /> 63 N Copies of hazardous waste disposal records for the parts washer waste <br /> were not found on site. <br /> Records indicate that the Parts washer is serviced twice a year by Safety- <br /> Kleen. Invoices and description of service for 2009, 2010 and 2011 to date, <br /> have been provided. Waste disposal records for the parts washer prior to <br /> R 2008 continue to be researched and located as OMI, Thames Water, a <br /> private company was under contract to operate and maintain this facility <br /> from 2002 through March 2008. Will continue to provide follow-up until <br /> research is complete. <br /> 64 V 64. Failed to retain copy of manifests or bill of lading for spent lead acid <br /> batteries. <br /> N Copies of spent lead acid battery disposal records were not found on site. <br /> R Waste disposal records for spent acid batteries are being researched and <br /> located. Will continue to provide follow-up until research is complete. <br /> 79 79. Failed to: (1) store UW lamps in a closed container and in a manner <br /> V that prevents breakage and release to the environment, or immediately <br /> clean and containerize any broken lamps or releases to the environment. <br /> N Used Fluorescent bulbs were observed stored open and uncontainerized in <br /> the Old Cogen Building. <br /> July 1, 2011 the fluorescent bulbs were sent over to MUD Stores and <br /> R properly signed over, using Mud Stores <br /> Fluorescent Lamp Collection And Disposal Log for proper disposal. <br />
