Failed to keep signed copy for 3 years. 61. Failed to determine status 61 V of hazardous waste when manifest copy not received. 62. Failed to submit & an exce tion report. 62 Manifest numbers 004907611JJK (3/26/09), 004907722JJK (7/21/09), N 004907827JJK (10/14/09), 004907917JJK (1/14/10), 006396647JJK (6/17/10), 006396849JJK (12/30/10) and 006661230JJK (3/24/11) were found without a signed off copy from the destination facility. Manifest numbers 004907611JJK (3/26/09), 004907722JJK (7/21/09), 004907827JJK (10/14/09), 004907917JJK (1/14/10), 006396647JJK (6/17/10), 006396849JJK (12/30/10) and 006661230JJK (3/24/11) are all Manifests for disposal of Non-RCRA Solids, oily waste that were transported R by Evergreen Environmental. Inquiries have been made Evergreen Environmental to locate the designated facility signed copy of the manifests. We have requested an update from Evergreen on their progress on locating these manifests. If they are unable to locate the Manifests by August 29tH Exception reports will be filed with DTSC. 60 V 60. Failed to keep signed copy for 3 years. 63. Failed to keep a copy of & the consolidated manifest for 3 years. 63 N Copies of hazardous waste disposal records for the parts washer waste were not found on site. Records indicate that the Parts washer is serviced twice a year by Safety- Kleen. Invoices and description of service for 2009, 2010 and 2011 to date, have been provided. Waste disposal records for the parts washer prior to R 2008 continue to be researched and located as OMI, Thames Water, a private company was under contract to operate and maintain this facility from 2002 through March 2008. Will continue to provide follow-up until research is complete. 64 V 64. Failed to retain copy of manifests or bill of lading for spent lead acid batteries. N Copies of spent lead acid battery disposal records were not found on site. R Waste disposal records for spent acid batteries are being researched and located. Will continue to provide follow-up until research is complete. 79 79. Failed to: (1) store UW lamps in a closed container and in a manner V that prevents breakage and release to the environment, or immediately clean and containerize any broken lamps or releases to the environment. N Used Fluorescent bulbs were observed stored open and uncontainerized in the Old Cogen Building. July 1, 2011 the fluorescent bulbs were sent over to MUD Stores and R properly signed over, using Mud Stores Fluorescent Lamp Collection And Disposal Log for proper disposal.
