Stacy Rivera <br /> August 3, 2011 <br /> Page 4 <br /> UPDATE ON RETURN TO COMPLIANCE CERTIFICATION - HAZARDOUS WASTE <br /> GENERATOR PROGRAM INSPECTION REPORT AND ABOVEGROUND <br /> PETROLEUM STORAGE ACT INSPECTION REPORT FOR CITY OF STOCKTON <br /> MUNICIPAL UTILITIES DEPARTMENT MAIN PLANT, TERTIARY PLANT AND <br /> STORES FACILITIES INSPECTIONS CONDUCTED MAY 11, 2011 <br /> 83 83. Failed to label universal waste to identify type of universal waste. (a) <br /> V Batteries, (b) Mercury containing equipment, (c) Lamps, (d) Electronic <br /> devices, e CRTs, and CRT glass. <br /> N Four 14 foot used fluorescent bulbs and a pile of electronic waste were <br /> observed unlabeled in the Old Cogen Building. <br /> All E-WASTE was collected by Tri-Valley Recyclers on July 26, 2011. A sign <br /> was installed at the E-WASTE area. A second pallet has been provided <br /> R expand the collection area for E-Waste collected in the future. All <br /> Fluorescent bulbs were transported to MUD Stores for proper disposal. <br /> See Attachment 1 <br /> 84 Stored UW on site for more than 1 year, or unable to demonstrate the length <br /> V of time the UW has been accumulated from the date it became a waste or <br /> was received. <br /> A 5 gallon bucket of alkaline batteries was observed in the Old Cogen <br /> N building with an accumulation start date of December 2, 2008. The Facility <br /> was unable to demonstrate the amount of time used fluorescent bulbs and <br /> waste electronics in the Old Cogen Building were stored on site. <br /> All accumulated Used Batteries were picked up by Evergreen Environmental <br /> R on July 27, 2011. New a-waste labels with correct accumulation start dates <br /> have been placed on collection area See Attachment 2 <br /> 85 V 85. Failed to keep records of each shipment of universal waste according to <br /> this section. Addressed and under SJEHD Review. <br /> TERTIARY PLANT (3307 W. Hwy 4) —CAL000290827 <br /> 50 RV 50. Accumulated more than 6,000 kg of HW on site at one time. <br /> Addressed and under SJEHD Review. <br /> 59 V 59. Failed to send generator manifests copies to the Department within 30 <br /> days. Addressed and under SJEHD Review. <br /> 60 V 60. Failed to keep signed copy for 3 years. 62. Failed to submit an <br /> & exception report. <br /> 62 N Manifest Number 003360529FLE 7/22/10 was not found on site. <br /> Files are being searched for Manifest Number 003360529FLE (7/22/10) if <br /> R we are unable to find the Manifest by 8/29/11 we will file an Exception <br /> Report with DTSC. <br /> ABOVEGROUND PETROLEUM STORAGE ACT (APSA) INSPECTION <br /> MAIN AND TERTIARY PLANT APSA INSPECTION <br /> 8 8. Failed to amend SPCC Plan within 6 months, or implement changes <br /> V within 6 months of amendment when there is an applicable change to the <br /> facility <br /> N None <br /> R June 2011 Draft Working Copy incorporates all necessary changes or <br /> provides place holders for information updates. <br />
