1• <br />.0 <br />• • 0 <br />E <br />31 FIELD PERSONNEL- TRAINING AND MEDICAL REQUIREMENTS <br />Project personnel involved in initial installation of the system must meet the following <br />trairuing/ medical requirements <br />HAZWOPER 40 Hour Initial Training, <br />HAZWOPER 8 Hour Refresher Training (Current- i e , within last 12 months), <br />Medical Clearance Certificate (e g , from last annual/ biennial physical) <br />At least one on-site employee must meet the following requirements <br />• HAZWOPER 8 Hour Supervisor Certificate, <br />• Current certification in First Aid and CPR <br />Personnel using respiratory protection must meet the following requirements <br />Respirator Fit Test Record less than 12 months old (For APR), <br />Medical Clearance to wear a respirator (flus is normally on the Summary Profile for the <br />annual physical) <br />4.0 PROJECT DESCRIPTION <br />The site is located at 2651 South Airport Way in Stockton, CA The work area is on the grounds of <br />a baking plant, and is mainly covered by asphalt and concrete A CPT direct -push drilling rig <br />will be used to perform borings for soil and groundwater sample collection Work is planned <br />for May 15, 1999, and is expected to take 2 days <br />Site Health and Safety Plan <br />Earthgrains/Stockton, COL -99-011 <br />Stockton, CA <br />May 150, 1999 <br />2 <br />
