Ip <br />Mosquito Abatement <br />Page 2 r. <br />PHS/EHD filed a Notification of Hazardous Waste Discharge(Prop 65) on February 3, <br />1992 stating that the site was placed into the Local Overs h <br />g t Program (LOP) and <br />that additional site assessment was required to deterrr ° ne the nature and extent of <br />contamination. Subsequently, on February 10, 1994, you received notification that <br />the site was placed into the LOP and that additional soil remediation would be <br />necessary. PHS/EHD also suggested that the dry monitoring wells could be used for <br />vapor extraction and requested that an additional monitoring well be installed. <br />A fourth monitoring well was installed on June 14, 199 11 <br />f 2 in a suspected downgradient <br />direction. While groundwater contamination was not detected, this well is located <br />approximately 100 feet away from the area where soil contamination is evidenced. <br />During July 1994, additional soil and groundwater investigation occurred and further <br />evidenced soil contamination to 50 feet below ground s"urface. Also monitoring wells, <br />MW2 and MW3 were destroyed. <br />1 <br />You stated that'barriers to the investigation of the former tank area were the newly <br />installed tanks monitoring system which reportedly is encased in fiberglass conduit <br />without any metalized markings for detection. i'� <br />PHS/EHD also stressed the importance that the laboraf ory used for analysis achieve <br />approved detection limits for soil and groundwater as outlined in regulatory <br />guidelines. <br />PHS/EHD indicated that a summary of all the prior soil -and groundwater analytical <br />results from the tank removals, over -excavation, soil bo1rings;!'groundwater monitoring <br />wells and hydropunches should be prepared and submitted immediately. As was <br />agreed during our meeting a corrective action plan, as outlined in the California Code <br />of Regulations, necessary to remediate the residual soil" contamination which poses a <br />threat to groundwater, shall be submitted by April 15, 1995. 'Also, you may be asked <br />to install additional monitoring wells in the area of the former�jtank pit to replace wells <br />which were destroyed. <br />I.
