(ECM) <br />is submitting the enclosed boring permit application for the Membrane Interface Probe (MIP) <br />Investigation and ISCO Injection Project at the California Army National Guard Field <br />Maintenance Shop #24 located in Stockton, California. ECM has included the following as part <br />of the application package: <br />• Completed San Joaquin County — Environmental Health Department (SJEHD) Boring <br />Permit Application, <br />• Completed Master File Record Information (MFR) form <br />• Brief work scope summary <br />• Boring plan, <br />• Email approval of the Work Plan from the Central Valley Regional Water Quality Control <br />Board (CVRWQCB), and <br />• Check for $500.00 to cover the Boring Permit Fee ($125) and Inspection Fees ($375). <br />Per our discussion, you agreed to waive the Site Mitigation Fee and only charge ECM for the <br />Boring Permit Fee and Inspection Fees. In addition, you requested that ECM only submit a brief <br />work scope document and boring plan rather than the full approved Work Plan.
