(ECM)
is submitting the enclosed boring permit application for the Membrane Interface Probe (MIP)
Investigation and ISCO Injection Project at the California Army National Guard Field
Maintenance Shop #24 located in Stockton, California. ECM has included the following as part
of the application package:
• Completed San Joaquin County — Environmental Health Department (SJEHD) Boring
Permit Application,
• Completed Master File Record Information (MFR) form
• Brief work scope summary
• Boring plan,
• Email approval of the Work Plan from the Central Valley Regional Water Quality Control
Board (CVRWQCB), and
• Check for $500.00 to cover the Boring Permit Fee ($125) and Inspection Fees ($375).
Per our discussion, you agreed to waive the Site Mitigation Fee and only charge ECM for the
Boring Permit Fee and Inspection Fees.
