Anderson, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I, CLASS II, or MINOR-Notice to Comply) <br /> This is a revised inspection report. This version supersedes all previous versions. <br /> (continues from Page 5) <br /> 4. physical state <br /> 5. composition (contents) <br /> 6. accumulation start date <br /> Immediately label these containers and ensure that all hazardous waste containers are marked with all <br /> the required information. Based on disposal receipts and interview with generator, generator was able <br /> to demonstrated the accumulation start day of these waste were between a week and a month, and <br /> completed labeling of these containers. Violation was corrected on site. <br /> 701. (Repeat) Review of document and interview with generator indicated facility used to dispose waste <br /> oil once a year before 2012, with no disposal at all in 2009. Facilities who generate less than 1000 kg of <br /> hazardous waste per month and do not exceed 6000 kg of waste stored on site at any time may store <br /> waste on site up to 180 days. Immediately begin managing hazardous waste within 180 days per this <br /> section. <br /> 812. Universal waste disposal receipts for the last 3 years were not available for review during this <br /> inspection. A copy of the universal waste disposal records shall be retained on site for at least 3 years. <br /> Submit a copy of the disposal receipt from 2009 to current day, including the receipt of the universe <br /> waste lamps identified in the October 2011 inspection, to EHD by '11/4/2013. <br /> NOTES (following are not violations) <br /> -Facility generated 11.2325 tons of hazardous wastes in 2012. Facility has been re-classified as a 5-25 <br /> tons hazardous waste generator. <br /> -Assisted generator in creating CERS account and submitting some of the requirements (but not all) <br /> -Verified during this inspection that the following violations cited in the October 2011 Routine hazardous <br /> waste inspection report to have returned to compliance: <br /> 101. EPA ID number active <br /> 108. Wastes identified on the report have been determined as hazardous waste disposed under <br /> manifest in 2012. <br /> 126. Consolidated manifests information complete in years following the October 2011 inspection. <br /> 404. Containers closed. <br /> 407. Empty containers have been disposed and records kept. <br /> 605.
