Airport Way, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 28. According to Mr. Kitagawa, the containers in the satellite accumulation areas are being inspected <br /> quarterly. Inspection logs for daily inspections were provided for the containers in the Hazardous Waste <br /> Storage area. The owner or operator shall inspect areas used for container storage or transfer, at least <br /> weekly, looking for leaking containers and for deterioration of containers and the containment system <br /> caused by corrosion and other factors. Immediately begin conducting the necessary weekly inspections <br /> for all hazardous waste containers at this facility. <br /> 45. The contingency plan for this facility stated that a facility diagram with evacuation routes and building <br /> exits, the blank notification page submitted to local agencies, and a map showing the locations of <br /> emergency equipment were attached. None of these documents could be located at the time of <br /> inspection. Copies of the completed local agency notification letters were provided at the time of <br /> inspection. The contingency plan did not include the treatment unit and process. A contingency plan <br /> shall include all of the following: <br /> -actions facility personnel shall take in response to fires, explosions, and unplanned release to air, soil, <br /> or surface water <br /> -arrangements agreed to by local police departments, fire departments, hospitals, contractors, and state <br /> & local emergency response teams <br /> -names, addresses & phone numbers (office & home) of the Emergency Coordinator <br /> -list of all emergency equipment at the facility (fire, spill, communications/alarms, decontamination), <br /> including location, physical description, and a brief outline of its capabilities <br /> -evacuation plan, including routes and signals to be used <br /> -telephone number of Cal EMA <br /> This information must be kept current at all times. Immediately update the contingency plan for this <br /> facility to include all of the necessary information and submit a copy of the updated plan to the EHD by <br /> August 14, 2011. REPEAT VIOLATION <br /> 49. The training program for this facility was focused on enforcement and did not completely address <br /> hazardous waste management procedures and contingency plan implementation. Training is not <br /> conducted for the treatment unit and process. The training is being directed by Lon Kitagawa. Mr. <br /> Kitagawa's training was last conducted in November 2008 and is no longer current. The employee <br /> training records do not include written job descriptions or a description of the training to be given to each <br /> employee.
