Airport Way, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> 49. (continued) Records of additional hazardous waste management training were not available for <br /> employees who were unable to attend the initial refresher training. At the time of inspection, <br /> documentation could not be provided to indicate that all employees had completed the annual refresher <br /> training. Job specific training was not provided for those employees responsible for additional waste <br /> handling duties, such as container and tank inspections and labeling. Facility personnel shall complete <br /> a training program that teaches them to perform their duties in a way that ensures the facility's <br /> compliance with Title 22 regulations. This program shall be directed by a person trained in hazardous <br /> waste management procedures and shall include the following: <br /> - hazardous waste management procedures relevant to the positions in which they are employed, <br /> including contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include: <br /> - name &job title of each employee <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training within <br /> six months of hire and at least annually thereafter. Submit a copy of the updated training program and <br /> records to the EHD by August 14, 2011. REPEAT VIOLATION <br /> 59. Manifest numbers 002708925FLE (1219110) and 008482636JJK {5123/11) were found on site at the <br /> facility but were not on record with the Department of Toxic Substances Control (DTSC). Within 30 days <br /> of each shipment, a generator shall submit a legible copy of each manifest used to DTSC. Immediately <br /> submit a copy of these manifests to DTSC and ensure that copies of all future manifests are submitted <br /> to DTSC within the required time frame. <br /> ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122). <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> EHD Inspector: Received By: Title: <br /> EHD 23-02-003 Rev /1011 <br /> CONTINUATION FARM <br />
