According to Mr. Chris <br /> Hansen, employees are provided with a PPE training covering the use of gloves, respirators, safety glasses, and <br /> hearing protection. This training was not listed in the hazardous waste records as part of the training required. <br /> Employee training records shall include: <br /> 1. the job title for each position at the facility related to hazardous waste management, and the name of each <br /> employee filling each job; <br /> 2. written job description for each position listed above; <br /> 3. written description of the type and amount of both introductory and continuing training that will be given to <br /> each person filing positions listed above; <br /> 4. records that document that the training has been given to and completed by facility personnel. <br /> Training records on current employees shall be kept until closure of the facility. Training records on former <br /> employees shall be kept for at least three years from the date the employee last worked at the facility. Ensure that <br /> employee training is properly documented. <br /> This is a Class II violation. <br /> 123 HSC 25143.10 Failed to provide information for>100 kg/mo recyclable material to claim the exclusion or exemption. <br /> MEK and SBA is being recycled in two distillation machines on site and all required information to claim the exclusion <br /> or exemption has not been submitted. The most recent CERS submittal, dated 1/14/15, stated 'yes'to the <br /> Recyclable Materials section, but submitted an incorrect, unrelated document in as the Recyclable Materials Report <br /> Documentation. To claim an exclusion or exemption from classifying this as a waste, the following information shall <br /> be submitted every two years: <br /> -name, site address, mailing address, and telephone number of the owner or operator of the facility that recycles <br /> the material; <br /> - name and address of the generator of the recyclable material; <br /> -documentation that the requirements of any exemptions or exclusions pursuant to HSC 25143.2 are met. <br /> Immediately provide all required information for every recyclable material that you wish to claim the exclusion or <br /> exemption and provide that information every two years. If the required information cannot be provided, the materials <br /> must immediately be handled as a hazardous waste. <br /> This is a Class II violation. <br /> Page 6 of 10 <br />
