Charter Way, Stockton Program: HW <br /> SUMMARY OF VIOLATIONS <br /> CLASS I,CLASS 11,or MINOR-Notice to Comply) <br /> 49. (continued) <br /> - hazardous waste management procedures, including contingency plan implementation <br /> - emergency procedures, equipment, and systems <br /> - key parameters for automatic waste feed cut-off systems <br /> - communications or alarm systems <br /> - response to fire, explosion, or groundwater contamination incident <br /> - shut down of operations <br /> The training program shall be completed by each employee within six months of hire, prior to <br /> unsupervised work, and shall be reviewed annually. Training records shall be maintained on site until <br /> closure of the facility and shall include- <br /> - name &job title of each employee <br /> - written job description for each position (job title) listed, including requisite skill, education, <br /> qualifications, and duties <br /> - description of training to be given to each position <br /> - records that training was completed for each employee <br /> Immediately prepare and implement a complete hazardous waste management training program for this <br /> facility in accordance with Title 22 regulations and ensure that all employees complete this training at <br /> least annually. Submit a copy of the training program and records the the EHD by May 21, 2011. <br /> 60, 61, & 62. Manifest numbers 001509980SKS (10116108), 001154687GBF (10125110), and <br /> 001154742GBF (12130110) were found without a signed off copy from the destination facility. Hazardous <br /> waste generators shall retain copies of all manifests signed off by the destination facility on site for three <br /> years and have them readily available for review. If the generator did not receive a copy of the manifest <br /> with the handwritten signature of the owner or operator of the facility to which the generator's waste was <br /> submitted within 35 days of the date the waste was accepted by the initial transporter, the generator <br /> shall contact the transporter and/or the owner or operator of the designated facility to determine the <br /> status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within <br /> 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an <br /> Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the <br /> generator explaining the efforts to locate the hazardous waste and the results of those efforts. <br /> Exception reports shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O.
