Training records shall be maintained on site until closure of the facility and shall include- - name &job title of each employee - written job description for each position (job title) listed, including requisite skill, education, qualifications, and duties - description of training to be given to each position - records that training was completed for each employee Immediately prepare and implement a complete hazardous waste management training program for this facility in accordance with Title 22 regulations and ensure that all employees complete this training at least annually. Submit a copy of the training program and records the the EHD by May 21, 2011. 60, 61, & 62. Manifest numbers 001509980SKS (10116108), 001154687GBF (10125110), and 001154742GBF (12130110) were found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 35 days of the date the waste was accepted by the initial transporter, the generator shall contact the transporter and/or the owner or operator of the designated facility to determine the status of the waste. If the generator did not receive a copy of the signed off copy of the manifest within 45 days of the date that the waste was accepted by the initial transporter, the generator shall submit an Exception Report that includes a legible copy of the missing manifest, and a cover letter signed by the generator explaining the efforts to locate the hazardous waste and the results of those efforts. Box 806 Sacramento, CA 95812-0806 ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE($122).
