Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />October 28, 2003 <br />arrived at LLNLS300 badging office around 8:00 a.m., I had already phoned Mr. Shih so that he could get here <br />at about the same time. Mr. Shih arrived about the same time I did. We proceeded through the badging <br />proceedure and waited for Mr. Terusaki and Ms. Folks to arrive. <br />An in -briefing was held in the former location for the badging office. A sign -in sheet was passed around <br />(Attachment E). <br />I began the in -briefing by explaining the purpose of the visit. I explained that the inspection involves: taking <br />photographs, reviewing and copying records, questioning personnel and inspecting hazardous waste handling <br />areas. I also explained the role of the CUPA representative, Mr. Shih, stating that while he was here as an <br />observer he could make not of any concerns he or his Department might have and discuss it with them after my <br />inspection had been completed. I then asked for and received consent from Mr. Lane. <br />I provided Ms. Folks with a written list of records and documents which I would want to review (Attachment F). <br />We then discussed the areas which I planned to inspect. I explained that I would be including generator areas <br />as well as the permitted facilities during this inspection. I added that I would inspect the following areas; <br />EWSF Bldg. 883, EWSF Magazine 816 and M 1-5, EWTF Bldg. 845-A/B, the open burn and open detonation <br />treatment facilities, and EWTF Bldg. 829. <br />I began my observations with the permitted EWSF Bldg. 883, which is identified in the Part B as a permitted <br />container storage area. Mr. Fisher was the main escort for this portion of my inspection. Mr. Tageson was <br />currently not available. This facility is divided into two storage areas. The two storage areas are separated by <br />a masonry wall. The 90 -day storage area is on the north side and the permitted container storage area is <br />located on the south side. The permitted facility can store containers of hazardous waste for up to one year. <br />The entrances to both areas are maintained locked. At my request Mr. Fischer unlocked the gate to the <br />permitted storage area. Mr. Anderson, conducted radiation monitoring prior to any entry into the permitted <br />storage area. After Mr. Anderson deemed it safe for my entry I began my inspection of this area. A decon <br />station equipped with an eyewash/shower is located inside, and immediately to the right of the entrance. <br />DTSC (3/95) Page 1 of 12 <br />
