Inspection Report <br />Lawrence Livermore National Laboratories <br />Site 300 <br />Inspection date - October 28 and 29, 2003 <br />entrance to Magazine 816. 1 asked Mr. Gallagher if he would open the spill kit. Mr. Gallagher cut the plastic tie, <br />which he uses to keep the container locked. He explains that this type of tie, once latched, cannot be opened <br />without cutting it. Inside the kit I observed spill control supplies for both liquid and solid spills. The date of the <br />last inspection of the spill kit was painted on the container. I observed a date, 0/09/02, was painted on the <br />outside of the container. Mr. Gallagher explained that he inspects the container quarterly. He said that he <br />places a new date on at the completion of that inspection. I observed no violations in this area. <br />There is a decon station located outside of Magazine 816, and another one down the hill near M-2. Mr. <br />Gallagher operated the safety eye wash and the shower of the first decon station by Magazine 816 just as we <br />were driving up. I could see that everything worked quite well. On our way down to M-2, Mr. Gallagher <br />operated the decon station there, which I observed also to be working quite well. <br />DOCUMENT REVIEW: <br />After leaving Bldg. 883, 1 began my document review with the manifests which are kept in Mr. Tageson's office. <br />For details and results of the manifest review, see section three of this report. <br />At the end of the day a short informal close out meeting was held to review my the days activities. <br />October 29, 2003 <br />Upon arrival at LLNL Site 300 1 was met by Mr. Terusaki and Dawn Chase. A short in -briefing was held in Mr. <br />Lanes office. Mr. Sadlacek was unavailable at this time. I went over the type of inspection with Mr. Lane. <br />then asked for and received consent to do the inspection from Mr. Lane. <br />I completed my field observations with the permitted treatment facility identified in the Part B as Bldg. 845-A/B. <br />When we arrived at Bldg. 845-B we were met by Mr. Adams, Explosive Technician for EWTF. Mr. Adams has <br />responsibilities for treatment operations in this area. I observed the burn cage, referred to as the thermal burn <br />unit, which treats form three and form four waste. I also observed the burn table, or burn pan, which treats form <br />two waste. There were no signs of vegetation around the area, and all gates were locked.
