. lnapedion Report <br />explanation submitted on July 6 & July 7, 2005, 1 contacted Mr. Terusaki on August 30, 2005 via an e-mail and <br />requested additional information regarding LLNL S 300 submittal/documents provided on July 7, 2005. See <br />attachment D. The requested information was: <br />1- An explanation of why hazardous waste manifest number 23440682 showed that there were 7 containers of <br />hazardous wastes shipped from LLNL site 300 location to Clean Harbor (San Jose) and not from Safety Kleen <br />location as LLNL Site 300 submittal stated on July 7, 2005; <br />2- An explanation of who was in control of the hazardous waste from January 31, 2005 to February 10, 2005. <br />The hazardous waste manifest number 23440668 showed that Safety Kleen Systems Inc (Transporter) EPA ID <br />number TXR 000050930 was in control of the hazardous waste and not Safety Kleen (Oakland); and <br />3- If LLNL site 300 had any documents to show when (date) Safety Kleen received the 7 containers of <br />hazardous waste. <br />On August 30, 2005, 1 received an e-mail from Ms. Olga Ligeti, LLNL, RHWM Shipping Team Leader, with <br />responses to the information requested on August 30, 2005. Ms. Ligeti responsed to the questions were in the <br />same order as listed above. See attachment D. <br />1- Ms. Ligeti stated in her e-mail that Safety Kleen wanted to return the waste to Site -300 on February 10, 2005. <br />However, the facility could not receive the waste back, as it would be over a year old upon its arrival back. <br />Because the manifest had been adulterated by SK's staff, Ms. Ligeti, stated that she re -manifested the waste to <br />the new intended destination since SK never accepted the waste. SK's rejection was written on February 10, <br />2005 on manifest 23440668. They then gave all copies of that manifest to the LLNL driver. LLNL transported <br />the waste on manifest 23440682 which was prepared to deliver the waste from Safety Kleen to Clean Harbors <br />(San Jose). <br />2- Ms. Ligeti stated also that Safety Kleen transportation held the waste for that period pending acceptance <br />resolution. The hazardous waste manifest number 23440668 showed that Safety Kleen Systems Inc <br />(Transporter) EPA ID number TXR 000050930 was in control of the hazardous waste and not Safety Kleen <br />(Oakland). <br />3 -SK -Oakland never signed accepting the waste. <br />Based on Ms. Ligeti's responses. I contacted Mr. Terusaki, via an e-mail and I asked him to provide the operating <br />records for the 7 hazardous waste containers in question. See attachment D. <br />Page 6 of 20 <br />
