(Repeat) Manifest number 008108566JJK was found without a signed off copy from the destination facility. Hazardous waste generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received confirmation of delivery. This information shall be submitted to: DTSC Report Repository Generator Information Services Section P.O. Box 806 Sacramento, CA 95812-0806 Immediately locate a copy of the missing manifest or prepare and submit the required information to DTSC. By 08/08/2013, submit to the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. 301. Witnessed oily spots on the floor of the maintenance building by the track. Facilities shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, soil, or surface water which could threaten human health or the environment. Immediately begin minimizing spillage per this section, and manage any waste generated from clean up according to Title 22 hazardous waste regulations. 404. The lids on the three used oil drums (see Violation #605 & 701 below) at MOW were loosely placed on top; therefore the containers were not close. All hazardous waste containers shall be closed at all times except when adding or removing waste that would prevent spillage if the container is lying on its side. Immediately close these containers and ensure all hazardous waste containers are closed when not adding or removing waste. 407. Witnessed two empty hazardous material 55 -gallon drum behind the MOW, and seven empty 55 - gallon drums outside the Product Area. Containers larger than five gallons that previously held a hazardous material shall be marked with the date they were emptied and managed within one year of being emptied. Immediately mark and manage all empty containers per this section. ALL EHD STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE.
