There must be at least one emergency coordinator on site or on call to <br /> coordinate emergency response measures, and the following information must be posted by a phone: the name and <br /> phone number of the emergency coordinator; location of fire extinguishers, spill control equipment, and if present, fire <br /> alarm; and the phone number of the fire department, unless the facility has a direct alarm. Immediately appoint an <br /> emergency coordinator and post the required information by a phone. A form is provided that can be used for this <br /> purpose. This is a repeat violation, Class II. <br /> 109 CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br /> Manifest numbers 003150904SKS, 003280905SKS, and 003324783SKS were not found on site. The generator initial <br /> copy of manifest number 003963641 SKS was found without a designated facility signed off copy. Hazardous waste <br /> generators shall retain copies of all manifests for three years or until a copy signed off by the destination facility is <br /> received. The signed off copy shall be retained and made readily available for review for three years. If the generator <br /> did not receive a copy of the manifest with the handwritten signature of the owner or operator of the facility to which the <br /> generator's waste was submitted within 60 days of the date the waste was accepted by the initial transporter, the <br /> generator shall submit a legible copy of the missing manifest, with some indication that the generator has not received <br /> confirmation of delivery. This information shall be submitted to: <br /> DTSC Report Repository <br /> Generator Information Services Section <br /> P.O. Box 806 <br /> Sacramento, CA 95812-0806 <br /> Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to <br /> the EHD a copy of the signed off copy of the manifests or a copy of the information submitted to DTSC. <br /> 117 HSC 25404(e)(4) Failed to report program data electronically. <br /> A submission to the California Electronic Reporting System (CERS) for the hazardous waste program has not been <br /> made. Beginning January 1, 2013, all businesses are required to submit all new(or any changes to existing) <br /> hazardous waste information online to the CERS at http://cers.calepa.ca.gov. Be sure to include your hazardous <br /> waste activity in the Businesses Activities section in CERS in addition to any other relevant activities and required fields <br /> This is a Class II violation. <br /> 301 CCR 66262.34(d)(2) Facility not maintained to minimize the release of a hazardous waste. <br /> At the hazardous waste storage area there are dark oil stains and contaminated absorbent on the concrete. Facilities <br /> shall be maintained and operated to minimize the possibility of a fire, explosion, or release of hazardous waste to air, <br /> soil, or surface water which could threaten human health or the environment. Immediately clean up the dark oil stains <br /> and contaminated absorbent on the concrete of the hazardous waste storage area, and manage according to Title 22 <br /> hazardous waste regulations. Submit a statement and supporting documentation explaining how this waste was <br /> managed.
