Site Safety Plan • •
Job No. 4780201 Pa a 14
1
B. Environmental Emergency (Non-injury)
' An environmental emergency is a non-injury event that involves the release, or
potential release, of hazardous substances to the environment. In the event of
' an environmental emergency, personnel encountering a hazardous situation will
instruct others on site to evacuate the vicinity IMMEDIATELY. Immediately
telephone the Health and Safety Manager, Ken Price. In the event that the
' Health and Safety Manager is unavailable, contact the City of San Jose Fire
Department at 911 and report the incident. UNDER NO CIRCUMSTANCES
' should any individual re-enter an area in which hazardous substances have
been released, nor should an attempt be made to clean up a release until
instructions are given by qualified individuals of E2C, Inc. or the City of San
' Jose Fire Department.
If the magnitude of an incident exceeds on-site capabilities of control, the site
' should be evacuated. The SSO should signal the need for immediate site
evacuation by sounding three short blasts on a vehicle or air horn. Personnel
will immediately leave the site using the most direct route and immediately
' meet in a predesignated area for a headcount. Decontamination will then take
place in this area.
' C. Personal Injury
' In the event of personal injury, call the pertinent number(s) listed on Page 13
for medical assistance. If a decision is made to transport an injured individual
to a medical facility, instruct a person remaining on the site to telephone the
receiving medical facility and state the situation, including the nature of the
physical injury or chemical exposure (e.g., unconscious, bleeding, blinded by
' toxic vapors). Injured individuals should be transported to:
Dameron Hospital
' 525 West Acacia Street
Stockton, California
' (209) 944-5550
Nearest Cross Street: Lincoln
' The location of Dameron Hospital is shown on Figure 1A at the end of this
report. A copy of this SSP should be taken to the receiving medical facility
' with the injured person. Notify the SSO, Project Manager, or Health and Safety
Manager immediately of all accidents, incidents, or situations that resulted in
personal injury.
' EzC, Inc.
