Copies of hazardous waste disposal records for the last three years were not found on site. <br /> Hazardous waste generators shall retain copies of all manifests signed off by the disposal facility and all <br /> receipts used in a consolidated manifesting procedure on site for three years and have them readily <br /> available for review. Immediately locate a copy of all manifests and receipts for the last three years, <br /> maintain them on site, and submit copies to the EHD by May 5, 2012. <br /> 127. Copies of spent lead acid battery disposal records for the last three years were not available on <br /> site. The generator shall retain on site for at least three years a legible copy of each manifest or bill of <br /> lading which identifies spent lead acid batteries shipped to a person who stores, uses, reuses, recycles, <br /> or reclaims the batteries or their components. Immediately locate a copy of all disposal records for <br /> spent lead acid batteries for the last three years, maintain them on site, and submit copies to the EHD by <br /> May 5, 2012. <br /> 128. Bills of lading for the recycling of used oil filters for the last three years were not found on site. A <br /> copy of each bill of lading must be kept on site for at least three years. Immediately locate a copy of all <br /> bills of lading for used oil filters for the last three years, maintain them on site, and submit copies to the <br /> EH by May 5, 2012. <br /> 812. Copies of universal waste disposal records for the last three years were not found on site. A <br /> generator of universal waste (batteries, mercury containing equipment, lamps/bulbs, electronic devices, <br /> CRTs, CRT glass, aerosol cans) shall keep a record of each shipment of universal waste on site for at <br /> least three years. The record may take the form of a log, invoice, manifest, bill of lading, or other <br /> shipping document, and shall include the following: <br /> - Name & address of destination facility <br /> - Quantity &type of universal waste <br /> - Date of departure/shipment <br /> Begin this practice immediately. <br /> Complete and submit a copy of the Return to Compliance Certification to the EHD by May 5, 2012 along <br /> with a statement documenting the corrective actions that have been or will be taken for each violation <br /> and any supporting paperwork. <br /> ALL END STAFF TIME ASSOCIATED WITH FAILING TO COMPLY BY THE ABOVE NOTED DATES WILL BE BILLED AT THE CURRENT HOURLY RATE. <br /> THIS FACILITY IS SUBJECT TO REINSPECTION AT ANY TIME AT EHD'S CURRENT HOURLY RATE. <br /> END Inspector: Received By: Title: <br /> Stacy Rivera (209) 468-3440 <br /> EHD23-02-003 Rev07112111 CONTINUATION FORM <br />
