CLASS II. -r MINOR -Notice to ComoIV) <br />Item # <br />Remarks <br />109 <br />CCR 66262.40(a) Failed to keep signed copy of manifests from the designated facility for three years. <br />Manifest number 008680836JJK was found without a signed off copy from the destination facility. Hazardous waste <br />generators shall retain copies of all manifests signed off by the destination facility on site for three years and have them <br />readily available for review. If the generator did not receive a copy of the manifest with the handwritten signature of the <br />owner or operator of the facility to which the generator's waste was submitted within 60 days of the date the waste was <br />accepted by the initial transporter, the generator shall submit a legible copy of the missing manifest, with some indicatio <br />that the generator has not received confirmation of delivery. This information shall be submitted to: <br />DTSC Report Repository <br />Generator Information Services Section <br />P.O. Box 806 <br />Sacramento, CA 95812-0806 <br />Immediately locate a copy of the missing manifests or prepare and submit the required information to DTSC. Submit to <br />the EHD a copy of the signed off copy of the manifest or a copy of the information submitted to DTSC. A signed copy of <br />the report was provided to generator after searching on line as a one time. assistance. Generator stated he will begin <br />retaining signed off copy of all manifest for 3 years from now on. This is a minor violation and has been corrected on sil <br />112 <br />HSC 25160.2(b)(3) Failed to keep copies of consolidated manifesting receipts for three years. <br />Not all the consolidated manifest receipts for used oil and waste anti -freeze in 2013 were available. Generator shall reta <br />a copy of the consolidated hazardous waste manifest for last 3 years. Submit a copy of the missing receipts to EHD. <br />This is a minor violation. <br />601 <br />CCR 66262.34(d) Stored hazardous waste onsite greater than 180 days. <br />Stored two 55 -gallon drums of Used Oil, one 55 -gallon drum of waste anti -freeze, and one 5 -gallon bucket of brake <br />shavings longer than 180 days. Facilities who generate less than 1000 kg of hazardous waste per month and do not <br />exceed 6000 kg of waste stored on site at any time may store waste on site up to 180 days. Immediately contact a <br />licensed hazardous waste hauler to dispose of this waste under manifest and submit a copy of the manifest to the EHD. <br />This is a repeat violation, Class II. <br />605 <br />CCR 66262.34(f) Failed to completely label containers or tanks of hazardous waste. <br />5 -gallon bucket of brake shaving had no hazardous property on the label. All hazardous waste containers shall be <br />marked with the following information: <br />- the words "Hazardous Waste' <br />- name and address of generator <br />- hazardous properties <br />- physical state <br />- composition (contents) <br />- accumulation start date <br />Immediately label these containers and ensure that all hazardous waste containers are marked with all the required <br />information. This is a repeat violation, Class II.
