VII. PERMIT PROCEDURES <br /> 1. The permit is a written authorization and approval for work that <br /> requires entrance into a permit-required confined space (see <br /> Appendix A). <br /> 2. The permit specifies the permit space and type of work to be done. <br /> 3. The permit specifies the date and authorized duration of the entry <br /> permit. <br /> 4. The permit identifies each Authorized Entrant within the permit space. <br /> 5. The permit identifies, by name, the Attendant(s). <br /> 6. The permit identifies, by name, the Entry Supervisor. <br /> 7. The permit identifies the hazards of the space, measures to isolate and <br /> eliminate or control hazards of the space before entry, acceptable entry <br /> conditions, results of initial and periodic tests, emergency procedures, <br /> communication procedures used by Entrants, and equipment to be <br /> provided. <br /> 8. Canceled permits will be used by the safety staff to review the permit- <br /> required confined space Program on an annual basis. <br /> 9. The Project Safety Officer shall be responsible for securing the permit <br /> from the Company Safety Officer. <br /> • The permit should be obtained at least 24 to 48 hours in advance of <br /> entry so that all pre-entry requirements can be completed and the entry <br /> conducted on schedule. <br /> 10. The Company Safety Officer or Branch Safety Officer shall review each <br /> portion of the permit with the Entry Supervisor. <br /> ADNMS/CONSP 10 June 2. 1994 <br />
