• All items which are marked with a "Y" or yes in the box provided are <br /> established as a requirement. <br /> All items which will not be required will be marked as NA (not <br /> applicable) or NR(not required). <br /> • The Branch/Company Safety Officer will sign the form at this time. The <br /> signature establishes the requirements that the Project Safety Officer <br /> must complete prior to/during entry. The Project Safety Officer shall <br /> not change any requirements specified by the Branch/Company Safety <br /> Officer. <br /> 11. The permit space entry permit has been designed to be used as a <br /> checklist for the Project Safety Officer. <br /> • The Project Safety Officer takes the permit and arranges for all required <br /> items on the checklist to be completed. <br /> 12. The Project Safety Officer is responsible for all aspects of the entry of <br /> one or more PACIFIC employees or contractor personnel into a permit <br /> space. <br /> • The Project Safety Officer shall determine that each requirement of the <br /> permit has been completed prior to confined space entry. The Project <br /> Safety Officer shall sign the permit thereby documenting that all <br /> requirements have been met. <br /> • The Project Safety Officer shall be present during the permit space <br /> entry. It is not necessary for the Company Safety Officer or Emergency <br /> Response Team to be in attendance. <br /> • The Project Safety Officer shall reauthorize entry whenever a re-entry is <br /> made; such as after breaks or lunch. <br /> 13. The permit must be completed in its entirety including all necessary <br /> signatures for a permit space entry to begin. <br /> • The permit shall be posted in a conspicuous place near the confined <br /> space entrance point and protected from weather. <br /> ADMMS/CONSP 11 June 2, 1994 <br />
