An approved copy of the monitoring plans were not available for inspecticn. The existing monitoring plan was not current and/or not approved by the EHD. Statistical inventory reconciliation was identified on Diesel TA0235201 tank form, according to Mr. Mike Boparai, statistical inventory reconciliation is not being preformed for this tank. The corrected monitoring plan was uploaded to the California Environmental Reporting System (CERS) at the time of inspection. This was corrected on site. This is a Class II violation. 209 CCR 2715(c) Designated operator did not inspect all required items and/or the inspection reports not completed. 1)The designated operator failed to check and note the correct test dates for the routine secondary containment testing on all 12 most recent designated operator inspection reports. According to EHD's records this test was conducted on 8-3-2016, however, it was listed on all above reports as 8-24-2016. During the monthly inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. 2)The designated operator failed to document the following alarms: -1-17 alarm (occurred 6-27-2017)was not documented on the 7-31-2017 inspection report (the prior inspection report was dated 6-22-2017) -1-24 alarm (occurred on 8-21-2017)was not documented on the 8-28-2017 inspection report (the L24 alarm was documented, however, the date it alarmed was missing) -T6 overfill(occurred on 8-31-2017)was not documented on the 9-23-2017 inspection report. -L11 alarm (occurred on 10-19-2017)was not documented on the 10-27-2017 inspection report. -L1 alarm (occurred on 10-25-2017) and 1-11 alarm (occurred on 10-19-2017)were not documented on the 10-27-2017 inspection report. -L9 alarm (occurred on 12-27-2017), L17 alarm (occurred on 12-27-2017), L21 alarm (occurred on 12-27-2017), L22 alarm (occurred on 12-27-2017), and L25 alarm (occurred on 1-16-2018)were not documented on the 1-25-2018 inspection report. -L25 alarm (occurred on 2-1-2018)was not documented on the 2-28-2018 inspection report, also, the alarm history print out attached to this inspection report appeared to be in a different language and was not legible. -L1 alarm (occurred on 3-28-2018)was not documented on the 3-28-2018 or the 4-26-2018 inspection reports. During the monthly inspection, the designated operator shall review the alarm history for the previous month, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm history with documentation taken in response to any alarms to the monthly report. Ensure that designated operators performing monthly inspections at this facility are including all of the required information on the reports. Submit proof of correction to the EHD.
