• Medical and occupations history <br /> • Height, weight and blood pressure <br /> • Vision test <br /> • Urinalysis <br /> • Audiogram <br /> • Blood analysis of 32 blood parameters(SMAC 32) <br /> • Urinalysis and/or blood testing for baseline heave metals(lead) <br /> • EKG, resting <br /> • Chest X-Ray <br /> • Spirometry <br /> Based on this examination, the physician has certified whether the individual is capable of full <br /> participation in the program, or whether he or she must work within certain restrictions. <br /> Any person exposed to high levels of hazardous substances are required to undergo a repeat medical <br /> examination at or before the conclusion of the project to determine the medical implications of this <br /> exposure. If air monitoring results indicate airborne levels above established guidelines, the physician <br /> will be notified to review present physicals and determine if levels warrant additional physical <br /> examinations. <br /> All medical records are held by Ogiso Environmental for a period of at least 30 years after the <br /> employee's termination of employment, in accordance with OSHA regulations on confidentiality and <br /> record keeping. <br /> In addition to the physical examination prior to entering the work zone, on site medical surveillance is <br /> standard Ogiso Environmental procedure and includes heat stress monitoring and prevention, <br /> containment monitoring, and hearing conservation measures. <br /> 9.1 Respirator Fit Testing <br /> a. No employee shall use an air purifying respirator,including stand-by mode, <br /> unless the respirator, by manufacture, make, model and size has been <br /> properly fated to the employee. Proper fit shall be determined by a <br /> qualified fit test. <br /> b. Employee qualitative fit tests for air purifying respirators shall be <br /> performed by an industrial hygienist or person having similar education, <br /> training and experience. <br /> C. Fit tests shall be performed annually, preferable at the time of training. <br /> _ 10.0 Medical Emergency <br /> 10.1 Emergency Safety Equipment <br /> The first aid kit and emergency eye wash will be located in Ogiso Environmental truck. All Ogiso <br /> Environmental Project Multimedia Standard First Aid Kits are OSHA approved. <br /> 10.2 Emergency Warning System <br />
