During the project, there will be an air ham located in the Decontamination Reduction Zone of <br /> the site. <br /> One long blast will be used for emergency evacuation of the exclusion zone. The Contamination <br /> Reduction zone will be the rally point. After a head count, farther instructions will be given. <br /> Repeated short blasts will be used for emergency evacuation of all personnel from the site. Ogiso <br /> — Environmentals truck will be the rally point. After a head count, further instructions will be given. <br /> 10.3 Accident Reporting System <br /> All field personnel are instructed to report accidents to the Project Supervisor and Site Safety <br /> Officer. Within Ogiso Environmental.,the accidents will be handled in accordance with the <br /> guidelines set forth in the Injury and Illness Prevention Plan which is in fall compliance with <br /> California's SS 198, effective July 1, 1991. <br /> 11.0 Emergency Decontamination <br /> The decision whether or not to decontaminate a victim by on-site staff will be based on the type and <br /> severity of the illness or injury. If necessary, based on the severity of the injury, decontamination will be <br /> conducted by emergency medical technicians called to the site. <br /> • To decontaminate the victim, the victim's protective equipment will be washed, <br /> rinsed and/or removed or cut off. <br /> • If decontamination is not allowed by emergency medical technicians called to <br /> the site,the victim will be wrapped in a blanket or Visquene to minimizz <br /> possible contamination of other personnel. <br /> Emergency medical personnel will be notified of potential contamination and <br /> proper decontamination procedures. <br /> • An Ogiso Environmental employee familiar with the conditions and the <br /> incident will accompany the victim. <br /> 11.1 Emergency Telephone <br /> The portable telephone in Ogiso Environmental truck will act as the emergency telephone. <br /> Instructions for emergency calls and emergency numbers will be posted near the telephone. <br /> 11.2 Emergency Telephone Numbers <br /> • Ambulance 911 <br /> _ • Fire 911 <br /> • Hospital 911 <br /> • Police 911 <br /> 11.3 Route to Hospital <br /> Site Safety Officer <br /> Mr. Gene Fiedler is qualified for S.S.O. position based on his education,training and experience in the field of <br />
