The HSM is responsible for providing the appropriate <br />monitoring and safety equipment necessary for the implementation of this HSP. <br />Any significant changes to the HSP must be approved by the HSM. <br />3.2 Site Health and Safety Coordinator (SHSC) <br />The SHSC has the responsibility to implement this HSP. The SHSC will investigate <br />all accidents and incidents occurring at the site. The SHSC will conduct safety <br />briefings and site-specific training for on-site personnel. The SHSC will accompany <br />all OSHA and other governmental agency personnel visiting the site in response <br />to health and safety issues. He is responsible for modifying and/or developing new <br />procedures, after consulting with the HSM, when site or environmental conditions <br />change due to natural causes or due to the conduct of the site operations. <br />The SHSC has stop -work authorization if he determines that an imminent safety <br />hazard or potentially dangerous situation exists. Authorization to proceed with work <br />will be verified by the HSM. The SHSC will be responsible for implementing <br />evacuation procedures, including the shutting down of appropriate equipment, <br />removing equipment from areas and coordination necessary emergency services <br />on-site. <br />3.3 Team Members <br />All team members, including all employees of subcontractors working at the site, <br />will be responsible for understanding and complying with all site health and safety <br />requirements. All members of this group will have been provided any site-specific <br />training regarding the hazards and protection involved for this particular project. <br />Subcontractors will be responsible to provide their employees with appropriate <br />equipment and clothing. <br />4.0 DEMOLITION AND REMOVAL OF UNDERGROUND FUEL PIPING <br />The removal of above ground structures, equipment and surface materials related <br />to the underground fuel piping removal will be performed in compliance with all <br />state and local regulations. Appropriate action will be taken to avoid the spreading <br />of dust to occupied portions of the surrounding area. Water will be used for dust <br />control in such a manner as to not result in a hazardous or objectionable condition. <br />Compliance will be in accordance with the South Coast Air Quality Management <br />District. <br />During excavation and removal of the underground fuel piping, it is anticipated that <br />no persons will enter an open excavation. If it is deemed necessary appropriate <br />measures will be taken to protect personnel when entering any excavation below <br />five (5) feet in depth. These measures will include sloping or shoring the side walls <br />of the excavation to protect individuals from hazards associated with cave-ins per <br />
