)L-IJESTIC TECH <br /> 9 . 0 GENERAL HEALTH AND SAFETY <br /> 9. 1 TRAINING <br /> All field personnel, including subcontractor personnel. assigned to this <br /> project shall satisfy the ttraining requirements of 29 CFR 1910. 120 (e) . <br /> including; <br /> OSHA UO-Hour Hazardous Waste Operations Training, all <br /> Personnel. <br /> OSHA-8HOur Refresher Training. all personnel who completed <br /> OSHA UO-Hour Training more than 12 months Previously- <br /> OSHA-8-Hour Supervisor Training, required for the Field <br /> Supervisor and any other field personnel assigned supervisory <br /> duties during the project. <br /> In addition, the following training requirements shall be satisfied: <br /> All field personnel assigned to the project shall be informed <br /> of , and trained on. the content and application of the SHP. <br /> This training shall be completed before the start of the <br /> project and shall be documented on a Record of Training Form <br /> ( see Majestic Technologies , Inc. ' s Health and Safety Manual , <br /> section 1200) . All field personnel shall recieve a copy Of the <br /> SHP and sign a Safety and Health Flan Compliance Agreement <br /> upon completion of this training ( see Attachment 2 ) . <br /> The Safety and Health Officer shall recieve a minimum of eight <br /> hours of specialized training from the contract Certified <br /> Industrial Hygienist in the use and maintainance of the air <br /> monitoring equipment , interpretation of data required to <br /> implement the SHP, and in-depth coverage of the elements of the <br /> SHP. <br /> All required train-ink shall be documented and Majestic Technologies , <br /> Inc . shall maintain copies of all training certificates and Record of <br /> Training forms . Copies of training records shall be maintained <br /> on-site during the project. <br /> 9. 2 MEDICAL SURVEILLANCE <br /> All field personnel, including subcontractor field personnel, <br /> assigned to the project shall participate in a medical surveillance <br /> program as required by 29 CFR 1910. 210 ( f) . All field personnel <br /> shall have completed an initial or update medical examination <br /> whithin the last twelve months (unless for an individual employee. <br />
