MAJESTIC TECH <br /> the examining physician has specified an alternative frequency for <br /> upd*te examinations) . All field personnel shall be approved to wear <br /> air Purifying respiratory protective equipment as part of the medical <br /> evaluation. <br /> Majestic Technologies, Inc. shall maintain curent records of <br /> participation in a medical survelliance program for each field person <br /> assigned to the project. This record shall include a copy of the <br /> physician ' s opinions, findings , and reccommendations ( including <br /> approval to wear respiratory protective equipment) for the most recent <br /> medical examination. A copy of these records shall be maintained <br /> on-site durin$ the project . Subcontractor personnel shall be <br /> responsible for administering a medical survelliance program for their <br /> employeed, and to provide approriate records of participation for <br /> subcontractor personnel assigned t0 the project. <br /> 9. 3 HAZARD COMMUNICATION <br /> Majestic Technologies, Inc, maintains a Hazard Communications program <br /> for its employees (see Majestic Technologies, Inc. ' s Health and <br /> Saefety Manual, section 1042) . All field Personnel assigned to the <br /> Project shall have received HBZard Communication training prior t0 the <br /> Start Of this project (i. e. , included in OSHA 40-Hour Hazardous waste <br /> Operations Training) , If avavailable, Majestic Technologies, Inc , <br /> shall provide Material Safety Data Sheets (MSDSs) representing the <br /> chemical substances being remediated under the project. No hazardous <br /> materials (as regulated by 29 CFR 1910. 1200) will be utilized during <br /> this project. If a change in the scope of work requires the use of <br /> hazardous materials, Majestic Technologies, Inc. shall provide and <br /> maintain copies of applicble MSDSs on-site. <br /> 9. 4 RESPIRATORY PROTECTION PROGRAM <br /> Majestic Technologies, Inc. maintains a Respiratory Protection Program <br /> for its employees (see Majestic Techrnolgoies . Inc. Health and Safety <br /> Manual, section 1043 ) . This program includes written procedures , <br /> training, medical surveillance, fit testing, maintenance equipment and <br /> Other components, All field Personnel assigned to this project shall <br /> be covered under this program (or an equivalent program for <br /> subcontractor personnel ) . This project will not require the use of <br /> supplied air respirators, <br />
