Provide rinsate manifest. <br /> 4. Sample 2 ft. to 4 ft. below tanks invert as per PHS-EHD sample <br /> protocol. <br /> 5. Obtain a new authorization to release signed and dated by tank <br /> owner. <br /> 6. Contractor must have third party sampler to obtain soil samples <br /> from the excavation and stockpile. <br /> 7. Obtain fire permit. <br /> S. Pay state surcharges. <br /> 9. Read and follow General Information (in tank removal plan) . <br /> May 18, 1995 <br /> Kasey Foley phoned around to local contractors to see if anyone else had bid <br /> on the 2523 E. Scotts job. Kasey talked to a local contractor who bid $3,300 <br /> to obtain permits and remove UST. When the contractor called Mrs. Romero to <br /> see who got the job, he was informed someone who bid less money to remove the <br /> UST got the job. <br /> May 19, 1995** As per Kasey Foley, write D.A.s' case. <br /> May 22, 1995 <br /> David Fisch came in to PHS-EHD and submitted the Authorization to Release <br /> signed by Sherry Romero, owner.
