Pg. 2 <br /> 5. Each worker assigned a respirator is expected to maintain it in good condition & to routinely <br /> inspect it before and after each use. Worn parts should be replaced immediately. Replacement <br /> parts will be available in the welding office. <br /> 6. Any malfunction of a respirator shall be reported to Van Holman. He will have special <br /> instructions regarding the technical aspects of the respirators. <br /> 7. Cartridges & Flter/Prefilters will be replaced when any odor, taste, or irritation is noted by the <br /> wearer. Also, they will be replaced if the breathing process becomes laboress or impaired. <br /> Proper fitting of all respirators is essential if the user is to be protected. An improper fit allows <br /> contaminated air to pass around the edges of the respirator, rather than through the filtering system. To <br /> make a good face seal, the user should take the following steps: <br /> 1. Place the respirator & straps in the appropriate wearing position. When adjusting head bands, <br /> pull the free ends tight until you get a comfortable fit. Secure all straps. <br /> 2. To adjust the face piece properly, put your chin firmly in the chin cup & shift the rubber mask <br /> until you find the most comfortable fit. Make final adjustments in the head band, but do not break <br /> the nasal seal. Do not modify the respirator or straps in any way. <br /> 3. Check for proper fit each time you wear the respirator, following the manufacturer's instructions. <br /> So not attempt to wear a respirator when projections under the face piece prevent a good face <br /> seal. Examples of projections include beards, sideburns, & temple pieces on glasses. <br /> 4. Testing of the respirator for a proper original fit will be accomplished with the use of isoamyl <br /> acetate - more commonly known as "Banana Oil." <br /> 5. If the employee is unable to obtain a satisfactory fit with the type of respirator which is provided, <br /> the company will correct the situation. <br /> Training in the use, Maintenance, & cleaning of all respirators will be implemented as follows: <br /> 1. Each employee who will use a respirator will be trained in its use maintenance. <br /> 2. The employees who will train aJl other employees will be Plant Personnel Bob Lind & Respirator <br /> Maintenance Lead-Man Van Holman. Bob Lind & Van Holman will receive detailed instructions <br /> on the maintenance & cleaning of respirator equipment which will be provided by our Insurance <br /> Company, Industrial Indemnity, who will send their Certified Industrial hygienist to provide us with <br /> the necessary technical information. <br /> 3. Employees will become familiar with the details of our program both through personalized <br /> instructions, & by reading the information provided in the written program. Copies of our program <br /> will be placed in the areas where the hazardous operations are performed. Also, a main file will <br /> be in place in the office along with our other safety records. Each respirator user will also receive <br /> a copy of this program for their personal use. <br /> 4. To insure proof of having received training, instructions, & copies of LMTs Respiratory <br /> Protection Program, after receiving all of the above, the employees involved will be required to sign <br /> the following declaration: <br /> As an employee of Lodi Metal Tech., Inc., I have received adequate training & instructions.in the <br /> selection, use & care of a respirator suitable for protection against airborne contaminants in my <br /> work assignments. I understand the elements of the respiratory protection program, & will apply <br /> them in the daily use, care, & safekeeping of the respirator equipment which is assigned to me. <br /> Signed -- Said Employee —� <br />
