6. You are responsible for securing excavation and stock piles if not backfilled immediately. <br />7. Submit the following information to EHD within 30 days after the UST removal date: <br />(a) Only analytical results that come directly from a third party California. certified <br />laboratory with affixed chain of custody will be accepted by EHD. <br />(b) UST Tracking Record Sheet and/or Tank Hazardous Waste Manifest. <br />(c) A copy of the completed Tank Closure Certification Form (DTSC Form 1249) <br />(d) A copy of the Uniform Hazardous Waste Manifest of the rinsate and any hazardous <br />waste signed by the designated facility which receives the waste. <br />8. See Page 2 of the Permit Application for additional general conditions. <br />9. This permit expires 180 days from the approval date. <br />2) New conditions: <br />1. EHD will take a sample of the brine to determine hazardous properties for our <br />independent evaluation <br />2. You are required to make a hazardous waste determination of any waste generated at <br />the site and dispose of the waste accordingly, or you may dispose of the waste as <br />hazardous waste under manifest to a permitted facility in accordance to Title 22. If <br />you choose to dispose of the removed brine as non -hazardous, please be cognizant that <br />it is a violation of Health and Safety Code (HSC) 25160(d) to transport hazardous <br />waste without a manifest, HSC 25163(a) to transport hazardous waste without valid <br />transporter registration, and HSC 25189.5(a) to dispose hazardous waste at an <br />unauthorized point. <br />3. City of Stockton Fire Department must be on site and give consent before the tanks <br />can be crushed. <br />The removal inspection with EHD has been scheduled for March 4, 2014. Please schedule at <br />least 48 hours in advance if there are any changes.
