SAN J O A Q U I N Environmental Health Department <br /> COUNTY <br /> Time In: 10:25 am <br /> �c Fawn` Greatness grows here. <br /> ' Time Out: 10:50 am <br /> Food Program Complaint Inspection Report <br /> Name of Facility: CHUCKS HAMBURGERS Date: 12/06/2018 <br /> Address: 5939 PACIFIC AVE, STOCKTON 95207 <br /> Owner/Operator: GRANT, STEVEN E Telephone: (209)473-9977 <br /> Program Element: 1600-FOOD PROGRAM Complaint#: C00047837 <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT ALLEGES ON 11/25/2018, SHE OBSERVED THE CEILING IS MISSING CEILING PANELS AND THERE <br /> IS A HOSE RUNNING ALONG THE CEILING. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Violations that are classified as"MAJOR"pose an immediate threat to public health <br /> and have the potential to cause foodborne illness.All major violations must be corrected immediately.Non-compliance may warrant immediate closure of <br /> the food facility. <br /> OVERALL INSPECTION NOTES AND COMMENTS <br /> FOOD ITEM--LOCATION--TEMP°F--COMMENTS <br /> No Temperature Data Collected <br /> NOTES <br /> Spoke with owner of the restaurant. He states the roof is being replaced by the owner of the building. Per owner all equipment <br /> and foods were kept free of contamination. The owner will keep the equipment and food from getting contaminated. The <br /> owner estimates the roof to be fixed within a week. <br /> There is no leaks at this time. The owner of the restaurant states the ceiling panels were removed because there was leak <br /> when it rained.The water was collected by plastic a containment system and piped into containers on the floor. The leak <br /> occurred in the dining area and part of the waitress area. <br /> The leak must be repaired withing a week.
